我国台湾地区医院出院准备服务的特点及启示

Characteristics and enlightenments of discharge preparation service of hospitals in Taiwan of China

为改善人民群众看病就医感受,国家卫生和计划生育委员会自2015年起在全国医疗系统开展"进一步改善医疗服务行动",并明确提出医院要完善入、出、转院服务流程,加强转院(科)患者的交接,提供连续医疗服务。文章通过介绍我国台湾地区医疗机构开展出院准备服务的背景、目的、意义、主要服务内容及其构建要素,以期为内地医疗机构推进出院准备服务改革,进一步改善医疗服务行动计划提供有益的参考。

To improve the feelings of the masses of people while seeing a doctor, the National Health and Family Planning Commission has launched the ＂Action Plan for Further Improving Medical Service＂ in the national health system since 2015 and clearly required hospitals to improve the service processes of admission to hospital, discharge from hospital, and hospital transfer, strengthen the handover of patients transferred from another hospital（department）, and provide continuous medical service. By introducing the background, goal, significance, main contents, and structuring elements of the discharge preparation service of medical institutions in Taiwan of China, the paper is expected to provide a useful reference for the mainland medical institutions to advance the reform of discharge preparation service and the Action Plan for Further Improving Medical Service.
