运动疗法联合针刺及推拿治疗慢性非特异性颈痛的效果观察

Effect of Physical Therapy Combined with Acupuncture and Massage Therapy on Chronic Nonspecific Neck Pain

目的:探讨运动疗法结合针刺、推拿治疗慢性非特异性颈痛的短期和长期效果。方法:108例慢性非特异性颈痛患者被随机分为观察组和对照组,两组患者均接受6周常规针刺、推拿治疗,前3周每周进行3次针刺、推拿治疗,后3周每周进行两次针刺、推拿治疗,观察组患者同时进行每周3次的颈椎运动疗法,对照组同时接受每周3次的磁热治疗。治疗前、治疗6周后及随访半年后,分别采用颈部功能障碍指数(NDI)、视觉模拟评分法(VAS)和颈部后伸肌群相对肌力来评价其颈部功能障碍程度、疼痛程度和颈部后伸肌群肌肉力量。结果:治疗6周后,观察组与对照组患者功能障碍、疼痛、颈部后伸肌群肌肉力量比较差异有统计学意义(P<0.05),这种疗效在随访半年后两组比较差异有统计学意义(P<0.05)。结论:颈椎运动疗法联合针刺、推拿对慢性非特异性颈痛患者的疼痛缓解、功能恢复具有更好的短期和较好的长期疗效。

Objective：To discuss the short-term and long-term effects of exercise therapy combined with acupuncture and massage on chronic nonspecific neck pain.Method：108 patients with chronic nonspecific neck pain were randomly divided into the observation group and the control group,patients in two groups were treated with 6 weeks routine acupuncture,massage therapy,in the first three weeks,received acupuncture and massage therapy three times a week,after three weeks,received acupuncture and massage therapy two times a week,patients in the observation group were also received cervical physical therapy three times a week,the control group received three times of magnetic therapy.Before treatment,treatment for 6 weeks and followed up for six months later,respectively used the neck disability index（NDI）,visual analogue scale（VAS） and neck extensors relative strength to evaluate the degree of cervical dysfunction,the degree of pain and the neck extensor muscle strength.Result：After 6 weeks of treatment,the observation group had a significantly better improvement in disability score,pain score and the muscle strength of neck extensor muscle（P〈0.05）.Significant differences between the two groups maintained at 6-month follow-up（P〈0.05）.Conclusion：Cervical physical therapy exercise therapy combined with acupuncture and massage therapy is effective in reducing pain and improving functional status and cervical stability in patients with chronic non-specific neck.
