B超引导下臂丛神经阻滞麻醉的临床效果评价

Evaluation on clinical effect of brachial plexus block anesthesia guided by B-mode ultrasonography

目的探讨B超引导下臂丛神经阻滞麻醉的临床效果。方法 60例上肢手术患者,采用随机数字表法、平行法分为观察组和对照组,每组30例。对照组患者予以盲穿法与腋路臂丛神经阻滞进行麻醉,观察组予以B超引导下联合腋路臂丛神经阻滞进行麻醉、观察患者麻醉效果、麻醉操作时间及不良反应。结果观察组患者的优良率为96.67%、显著高于对照组的80.00%,差异有统计学意义(P<0.05)。两组患者的操作时间比较,差异无统计学意义(P>0.05);观察组患者麻醉用药量显著少于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为3.33%,低于对照组的33.33%,差异有统计学意义(P<0.05)。结论在上肢手术麻醉中实施B超引导下联合臂丛神经阻滞麻醉的效果良好,可有效降低不良反应,操作简单,麻醉效果确切,值得临床广泛应用和推广。

Objective To explore the clinical effect of brachial plexus block anesthesia guided by B-mode uhrasonography. Methods A total of 60 upper limb surgery patients were divided by random number table and parallel method into observation group and control group, with 30 cases in each group. The control group received blind tees and axillary brachial plexus block for anesthesia, and the observation group received joint axillary brachial plexus block guided by B-mode uhrasonography for anesthesia. Observation were made on anesthetic effect, anesthesia operation time and adverse reactions in two groups. Results The observation group had higher excellent-good rate as 96.67% than 80.00% in the control group, and the difference had statistical significance （P〈0.05）. There was no statistically significant difference in operation time between two groups （P〉0.05）. The observation group had less anesthetic dose than that of the control group, and the difference had statistical significance （P〈0.05）. The observation group had incidence of adverse reactions as 3.33%, which was lower than 33.33% in the control group, and the difference had statistical significance （P〈0.05）. Conclusion Joint axillary brachial plexus block anesthesia guided by B-mode uhrasonography shows good curative effect in upper limb surgery anesthesia, which can effectively decrease the adverse reactions with simple operation and affirmative anesthetic effect. Therefore it is worth clinical wide application and promotion.
