摘要
目的观察动脉粥样硬化性脑梗死患者的红细胞分布宽度(RDW)水平变化,探讨其临床意义。方法选取2015年1月—2016年2月收治的90例动脉粥样硬化性脑梗死患者作为观察组,另选取90例健康体检者为对照组,观察血浆总蛋白(TP)、血浆前白蛋白(PA)、白蛋白(ALB)、球蛋白(GLB)、直接胆红素(DBIL)、总胆红素(TBIL)、低密度脂蛋白胆固醇(LDL)、总胆固醇(TC)、甘油三酯(TG)、胱抑素-C(Cys-C)等生化指标及红细胞计数(RBC)、血红蛋白(Hb)、红细胞比容(HCT)、平均红细胞体积(MCV)和RDW,及其与动脉粥样硬化性脑梗死的相关性。结果两组的高血压病、PA、TP、ALB、GLB和白蛋白球蛋白比值(A/G)比较差异有统计学意义(P<0.05,P<0.01);观察组RBC低于对照组,MCV、RDW明显高于对照组(P<0.01)。多因素Logistic回归分析结果示RDW是动脉粥样硬化性脑梗死的独立危险因素(P<0.001)。结论 RDW水平是影响动脉粥样硬化性脑梗死发病的重要因素,对于疾病预后评估有一定意义。
Objective To observe red cell distribution width (RDW) of patients with atherosclerotie cerebral infarction (ATCI) and to investigate its clinical significance. Methods A total of 90 patients with ATCI admitted during January 2015 and February 2016 were selected as observation group, and other 90 healthy people taking medical examina- tion were chosen as control group. Biochemical indexes such as plasma total protein (TP) , prealbumin (PA) , albumin (ALB) , globulin (GLB) , Direct bilirubin (DBIL) , total bilirubin (TBIL) , low density lipoprotein cholesterol (LDL) , total cholesterol (TC) , triglyceride (TG) and eystatin-C (Cys-C) , hemoglobin (Hb), haematocrit (HCT) , mean corpuscular volume (MCV) and RDW were compared in two groups, and correlations with otherosclerotic cerebral infarction were observed. Results There were significant differences in hypertension, PA, TP, ALB, GLB and albumin/globulin (A/G) (P 〈0. 05, P 〈0.01 ). In observation group, RBC level was significantly lower, while MCV and RDW levels were significantly higher than those in control group (P 〈 0.01 ). Multivariate Logistic regression analysis showed that RDW was an independent risk factor for atherosclerotie cerebral infarction (P 〈 0. 001 ). Conclusion RDW level is an important factor influencing pathogenesis of atheroselerotic cerebral infarction, and it has a certain significance for evalua- ting disease prognosis.
