腰椎动态固定及融合固定治疗腰椎退行性疾病的中短期随访

摘要 目的探讨腰椎动态固定及融合固定治疗腰椎管狭窄症和腰椎间盘突出症的中短期疗效。方法2008年7月至2014年6月,采用Wallis系统、Coflex系统以及腰椎融合系统,共治疗150例腰椎管狭窄症和腰椎间盘突出症患者。评价指标包括:体质量指数、视觉模拟评分法VAS、Oswestry功能障碍指数(ODI)和影像学分析。结果各组间VAS、ODI评分术前无统计学差异,术后组间VAS、ODI改善无统计学差异,显示不管是腰椎动态固定还是腰椎融合固定,在短中期的治疗效果是一致的。结论通过对比各组椎间盘Pfirrmann's分级,显示与腰椎融合固定组相比,Wallis系统、Coflex系统在保护手术节段及头侧邻近节段具有显著差异。 Objective To evaluate the short and medium term effectiveness of wallis,coflex and lumbar fusion system for treatment of lumbar spinal stenosis and lumbar disc herniation. Methods From July2008 to June 2014,150 cases cured with wallis,coflex and lumbar fusion system were included. The therapeutic effectiveness was evaluate by body mass index( BMI),visual analogue scales( VAS),Oswestry disability index( ODI) and disc Pfirrmann's type. Results The VAS,ODI of three group preoperation and postoperation were no statistical difference. The results were consistent in both the short and medium term,whether it was a dynamic fixation of the lumbar spine or a fusion of the lumbar spine. Conclusion By comparing the intervertebral disc Pfirrmann's score of each group,the results showed that,the protection of the surgical segment and the cephalic adjacent segment with Wallis system,Coflex system was significantly different compared with the lumbar fusion fixation group.
