超声造影对颈动脉溃疡斑块的诊断效能和一致性研究 被引量:5

摘要 目的探讨超声造影(contrast enhanced ultrasonography,CEUS)诊断颈动脉溃疡斑块的价值。方法对98处颈动脉斑块行彩色多普勒超声(CDUS)和CEUS检查,依据文献对颈动脉斑块形态进行分类,比较低年资和高年资医生应用CDUS和CEUS诊断颈动脉溃疡斑块的差异。结果低年资和高年资医生应用CDUS诊断颈动脉溃疡斑块的敏感性较低(30.00%,35.00%),而应用CEUS可明显提高诊断颈动脉溃疡斑块的敏感性(85.00%),且与CT诊断一致性较高(κ=0.9002),低年资和高年资医生间诊断差异无统计学意义。结论 CEUS诊断颈动脉溃疡斑块具有较好的准确性。 Objective To evaluate the value of diagnosis of ulceration plaque by contrast enhanced uhrasonograpy. Methods 98 carotid plaques were studied by colour Doppler ultrasonography (CDUS) contrast enhanced uhrasonograpy (CEUS). The mor- phology of carotid plaque was categorized according to references. The differences of diagnosis of ulceration plaque by a resident radiologist and a staff radiologist using CDUS and CEUS were compared. Results The sensitivity of diagnosis of ulceration plaque by both a resident radiologist and a staff radiologist was low (30.00% and 35.00% , respectively) using CDUS, however, the sensitivity of diagnosis of ulceration plaque by both a resident radiologist and a staff radiologist was higher ( both 85.00% ) using CEUS and the agreement of CEUS to CT was good ( k = 0. 9002). In addition, there were no differences in diagnosis of ulcera- tion plaque by both a resident radiologist and a staff radiologist. Conclusion There is a good accuracy in diagnosis of ulceration plaque by CEUS.
