复方藤梨根制剂联合XELIRI方案治疗结肠癌晚期肝转移的临床研究

Clinical Study on Patients with Advanced Liver Metastasis from Colon Cancer Treated with Fufang Tengligen Preparation(复方藤梨根制剂) Combined with XELIRI Chemotherapy Regimen

目的:观察复方藤梨根制剂联合XELIRI方案治疗结肠癌肝转移的临床疗效及其对免疫功能的影响。方法:将122例结肠癌肝转移患者随机分为观察组与对照组,观察组64例,对照组58例。对照组予XELIRI化疗方案(伊立替康+卡培他滨)治疗,观察组在对照组基础上服用中药复方藤梨根制剂。2组疗程均为12周,观察临床疗效,比较生存质量(KPS)评分、毒副反应及免疫相关指标的变化情况。结果:观察组、对照组总有效率分别为58.7%和36.2%,2组间比较,观察疗效明显优于对照组(P<0.05)。观察组、对照组生存质量评分改善率分别为59.4%、39.7%,差异具有统计学意义(P<0.05);观察组中性粒细胞减少程度较对照组减轻,差异有显著性意义(P<0.05);其它毒副作用2组比较,差异均无显著性意义(P>0.05);观察组治疗前后免疫指标CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+变化不明显,差异无统计学意义(P>0.05),对照组除CD_8^+外,CD_3^+、CD_4^+、CD_4^+/CD_8^+较治疗前明显降低,差异具有统计学意义(P<0.05);2组间治疗后比较,差异具有统计学意义(P<0.05)。结论:复方藤梨根制剂联合XELIRI方案治疗晚期结肠癌肝转移,临床疗效肯定,可改善患者的生存质量,减轻化疗毒副反应,改善免疫功能,起到增效减毒作用。

Objective：To observe the curative effect of Fufang Tengligen Preparation combined with XELIRI chemotherapy regimen on advanced liver metastasis from colon cancer.Methods：122 cases with liver metastases from colon cancer were randomized into observation group 64 and control group 58.Control group was treated by XELIRI chemotherapy regimens （Irinotecan ＋ apecitabine）,and observation group was treated by Fufang Tengligen Prepration additionally,with the course of 12 weeks.The clinical efficacy,KPS,toxic and side effect,immune indicators were observed.Results：The overall effective rate was 58.7% in the observation group and 36.2% in the control group,with a difference between the two groups （P 〈 0.05）.The improvement rate of KPS score was 59.4% in the observation group and and 39.7% in the control group,with a difference between the two groups（P 〈0.05）.The decreasing of neutrophils number was significantly lightened,the difference between two groups was significant （P 〈 0.05）,and no significant difference on other else.The T lymphocyte subsets （CD3＋,CD4＋,CD8＋,CD4＋/CD8＋） had no obvious changes in the observation group before and after treatment,CD3＋,CD4＋,CD4＋/CDs＋ in control group were lowered after treatment,significant differences were found in the levels of CD3＋,CD4＋,CD4＋/CDs＋ between the two groups （P 〈 0.05）.Conclusion：Fufang Tengligen Preparation combined with chemotherapy has better clinical curative effect on liver metastases of colon cancer,which can improve the patient＆#39;s survival quality,reduce the toxic and side effect,protect the body＆#39;s immune function,enhance therapeutic effect and reduce toxicity.
