血液PCT、hs-CRP、WBC、CD64联合检测在儿科感染性疾病中的应用价值 被引量:10

摘要 目的探讨血液PCT、hs-CRP、WBC、CD64联合检测在儿科感染性疾病应用价值。方法选取2015年7月~2016年10月我院儿科病房收治的117例感染性患儿为研究对象,根据患儿感染类型分为细菌性感染60例和病毒性感染57例,另选取同期健康体检儿童60例作为对照组,检测并比较三组血清PCT、hs-CRP、WBC、CD64水平。结果细菌感染组和病毒感染组患儿血清PCT、hs-CRP、WBC、CD64水平均明显较对照组高,且细菌感染组患儿血清PCT、hs-CRP、WBC、CD64水平较病毒感染组高,差异有统计学意义(P<0.05);细菌感染组血清PCT、hs-CRP、WBC、CD64及联合检测阳性率分别为86.67%、88.33%、76.67%、91.67%和96.67%明显较病毒感染组(21.05%、14.04%、17.54%、40.35%和42.11%)和对照组高(23.33%、8.33%、13.33%、28.33%和30.00%),差异均有统计学意义(P<0.05),病毒感染组血清PCT、hs-CRP、WBC、CD64及联合检测阳性率与对照组比较,差异无统计学意义(P>0.05)。结论血清PCT、hs-CRP、WBC、CD64联合检测有利于临床对患儿细菌感染性疾病的正确诊断,有助于临床对患儿感染性疾病分型的鉴别,从而指导临床用药具有重要意义。 Objective To investigate the application value of blood PCT,hs-CRP,WBC,CD64 in pediatric infectious diseases. Methods 117 cases of infectious children cured in pediatrics of our hospital from July 2015 to October 2016 were selected as the research object.According to the infection type,they were divided into 57 cases of bacterial infections and 60 cases of viral infection.60 healthy children were selected as control group.The serum PCT,hs- CRP,WBC,CD64 of the three groups were detected and compared. Results The serum PCT,hs-CRP,WBC and CD64 levels of bacterial infection group and viral infection group were significantly higher than those of control group; and the serum PCT,hs-CRP,WBC and CD64 levels of bacterial infection group were higher than those of viral infection group, and the difference was statistically significant(P〈 0.05).The serum PCT,hs-CRP,WBC,CD64 and the positive rate of combined detection of bacterial infection group were 86.67%,88.33%,76.67%,91.67% and 96.67%,compared with the viral infection group(21.05%,14.04%,17.54%,40.35% and 42.11%) and control group(23.33%,8.33%,13.3 3%,28.33% and 30%),the differences were statistically significant(P〈0.05).The positive rates of serum PCT,hs- CRP,WBC,CD64 and combined detection in the viral infection group were not significantly different from those in the control group(P〉 0.05). Conclusion Serum PCT,hs-CRP,WBC,CD64 combined detection is helpful for the correct diagnosis of bacterial infectious diseases in children.It contributes to the clinical identification of type of children infectious disease.It has important significance to guide the clinical medication.
