三点固定人工晶状体睫状沟悬吊术临床观察

Clinical observation on intraocular lens implantation by 3-point fixation ciliary sulcus suspension

目的评价微切口三点固定无巩膜瓣软性人工晶状体睫状沟悬吊术的临床效果及安全性。 方法对无晶状体且无完整晶状体后囊者20例（20只眼）采用3.0 mm切口以三点固定法（两点固定和另一缝线支撑）软性人工晶状体睫状沟悬吊术，术中不做巩膜瓣。随访6～24个月。 结果20例手术均顺利完成。术后1个月裸眼视力≥0.5者18例，占90.0%。于缝针穿过睫状沟时1例（5.0%）有少量出血，术后5 d吸收。无明显人工晶状体偏位，无视网膜脱离、角膜失代偿、继发性青光眼、严重葡萄膜炎、眼球塌陷或脉络膜下爆发性出血等严重并发症。 结论微切口三点固定无巩膜瓣软性人工晶状体睫状沟悬吊术是一种安全、有效的手术方法，效果良好。

Objective To observe the clinical efficacy and safety of soft intraocular lens implantation of 3-point fixation microineisional ciliary sulcus suspension without sclera flap. Methods Twenty eyes of 20 cases received soft intraocular lens implantation by a 3-point fixation （ two points fixed and a sutures supported） ciliary sulcus suspension with 3.0ram incision and without sclera flap. The follow-up time was 6-24months. Results The 20 cases of operationes were successful. One month after surgery, the uncorrected visual acuity 〉10.5 in 18 cases（90.0% ）. A small amount of hemorrhage caused by the sutures passes through the ciliary sulcus. The hemorrhage absorbed 5 days after surgery. There was no serious complication, such as IOL decentration or tilting, retinal detachment, corneal decompensation secondary glaucoma, severe uveitis, ocular collapse or expulsive choroidal hemorrhage. Conclusion Soft intraocular lens implantation by 3-point fixation microincisional ciliary sulcus suspension without sclera flap is an effective and safe method.
