摘要
目的探讨血糖水平与颅脑术后机械通气患者肺部感染的关系。方法选取2014-2016年5月我院综合ICU收治的60例接受颅脑术后机械通气治疗的患者,其中男性35例,女性25例,年龄在18-77岁之间,根据随机血糖水平将入组患者分为A、B、C三组。患者入院后常规监测血糖水平。平均4小时监测一次血糖水平。结果 A、B、C三组呼吸机相关性肺炎发生率分别为28.6%、19.2%、35%,其中B组(随机血糖10-12mmol/L)发生率最低,空腹血糖水平大于12mmol/L的C组呼吸机相关肺炎的发生率最高。差异均有统计学意义(P<0.05)。结论对于重型颅脑损伤或者大型颅脑手术后的患者来说,适度宽松的血糖控制水平反而更能使该部分患者获益。
Objective To investigate the relationship between blood glucose and ventilation associated pneu- monia in patients with mechanical ventilation undertaken craniocerebral surgery. Methods 60 patients with me- chanical ventilation after craniocerebral surgery from general ICU in our hospital were enrolled, of which 35 were male and 25 were female, aging from 18 to 77. All patients were randomly divided into the A, B, C groups. Patients ad- mitted were undergoing routine blood glucose detection, and all the patients ' blood glucose were detected every four hours. Results The incidence of ventilation associated pneumonia was 28. 6%, 19. 2% and 35% respectively in the groups A, B and C, and it was the lowest in the group B (blood glucose 10 -12mmol/L) and the highest in the group C (blood glucose above 12 mmol/L) (P 〈0. 05). Conclusion For patients with severe head injury or large craniocerebral surgery, moderately loose management of blood glucose can bring more benefits.
