热敏灸联合中药外敷治疗膝关节骨性关节炎临床研究

Thermal Moxibustion Combined with TCM External Application in Treatment of Knee Osteoarthritis

目的:探讨热敏灸联合中药外敷治疗膝关节骨性关节炎(knee osteoarthritis,KOA)的临床疗效。方法:选择KOA患者94例为研究对象,采用随机数字表法分为观察组和对照组,观察组患者给予热敏灸联合中药外敷治疗,对照组患者仅给予热敏灸治疗,两组均治疗3个月,比较并评价两组患者治疗前后膝关节Lysholm功能评分、疼痛视觉模拟评分法(visual analogue scale,VAS)评分、炎症指标,并对比两组临床疗效。结果:治疗后两组患者膝关节Lysholm功能评分、VAS评分均显著改善(P<0.05),且观察组患者膝关节Lysholm功能评分、VAS评分显著优于对照组(P<0.05);治疗后两组患者白细胞介素-1(interleukin-1,IL-1)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)及前列腺素-2(prostaglandin-2,PGE-2)水平均显著下降,且观察组IL-1、TNF-α及PGE-2水平显著低于对照组(P<0.05);治疗后观察组患者临床疗效显著优于对照组(χ~2=8.783;P=0.003)。结论:热敏灸联合中药外敷治疗KOA疗效显著,能够缓解患者疼痛、降低炎症,提高膝关节功能。

Objective：To explore the clinical efficacy of thermal moxibustion combined with TCM External Application in treatment of Knee Osteoarthritis. Methods：Ninety-four cases of patients with KOA in our hospital from April 2015 to August 2016 were selected as research objects and were randomly divided into observation group （48 cases） and control group （46 cases）. Patients in the observation group were treated with thermal moxibustion combined with TCM External Application, and patients in the control group were treated only with thermal moxibustion treatment. All treatments lasted for 3 months. The Lysholm function scores, visual analogue scale （VAS） scores,inflammation index and the curative efficacy of both groups were evaluated and compared before and after treatment. Results ： Lysholm function score and VAS pain score of the two groups after treatment were significantly higher than those before treatment （P 〈 0.05）. After treatment, the Lysholm function score and the VAS pain score of the observation group were significantly higher than those of the control group （ P 〈 0.05 ） ; Levels of interleukin-1 （ IL-1 ）, tumor necrosis faetor-α （TNF-α） and prostaglandin-2 （PGE-2） of the two groups after treatment were significantly higher than those before treatment （P 〈 0.05）. After treatment, the clinical efficacy of the observation group was significantly better than that of the control group （P 〈 0.05 ） and the level of IL-1, TNF- and PGE-2 of the observation group was significantly lower than that of the control group （χ^2 = 8. 783 ,P =0. 003 ）. Conclusion：The clinical efficacy of thermal moxibustion combined with TCM External Application in treatment of Knee Osteoarthritis is significant, which can relieve the pain, reduce the inflammation and improve the knee joint function.
