广泛切除联合无菌敷料打包植皮术治疗大面积肛周化脓性汗腺炎临床对照研究

Clinical treatment study of extensive excision and skin grafting dressing packing control in treatment of perianal suppurative hidrosadenitis of large area

目的分析广泛切除联合无菌敷料打包植皮术治疗大面积肛周化脓性汗腺炎的临床疗效。方法选取2014年8月~2016年3月收治的72例大面积肛周化脓性汗腺炎患者按随机数字表法分成观察组和对照组,每组各36例。对照组实行常规治疗,观察组采用广泛切除联合无菌敷料打包植皮术进行治疗。比较两组患者的治疗效果、感染发生率、复发率、恢复时间及治疗前、后生活质量。结果观察组治疗的总有效率明显高于对照组(P<0.05);观察组感染率及复发率均明显低于对照组(P<0.05);观察组疼痛消失时间、水肿消失时间、创面愈合时间及住院时间均明显较对照组短(P<0.05);两组治疗前生活质量各项得分及总得分均差异无统计学意义(P>0.05);观察组治疗后生活质量各项得分及总得分均明显低于对照组(P<0.05)。结论广泛切除联合无菌敷料打包植皮术能有效的提高大面积肛周化脓性汗腺炎的治疗效果,并降低感染率和复发率,缩短患者恢复时间,提高患者生活质量。

Objective Clinical analysis of hysterectomy combined with sterile dressing packing large area skin grafting in the treatment of perianal suppurative hidrosadenitis. Methods 72 cases of massive perianal suppurative hidrosadenitis patients from August 2014 to March 2016 were selected and randomly divided into observation group and control group, 36 cases in each group. Patients in control group were treated with routine treatment, and patients in observation group were treated with wide excision combined with sterile dressing packaged skin grafting for treatment. The treatment effect, the recurrence rate, infection rate, recovery time and before treatment, the quality of life of the two groups were compared. Results The total effective rate in observation group was significantly higher than that in control group （P 〈 0.05）. The infection rate and recurrence rate in observation group were significantly lower than those in control group （P 〈 0.05）; The pain disappeared time, edema disappeared time, wound healing time and hospitalization time in observation group were significantly shorter than those in control group （P 〈 0.05）. The scores and total scores of quality of life in the two groups before treatment were not significantly different （P 〉 0.05）. The quality of life scores and total scores in observation group after treatment were significantly lower than those in control group （P 〈 0.05）. Conclusion Wide excision combined with sterile dressing packaged skin grafting can effectively improve the large area of perianal suppurative hidrosadenitis treatment effect, and reduce the infection rate and recurrence rate, patient recovery time is shorter, and the qualitv of life is significantlv better.
