交锁髓内钉治疗四肢骨干严重粉碎性骨折的回顾性研究

Retrospective study of interlocking intramedullary nail in treatment of severe comminuted fracture of extremities

目的探讨交锁髓内钉治疗四肢骨干严重粉碎性骨折的临床价值。方法回顾性分析我院2013年8月~2015年11月四肢骨干严重粉碎性骨折患者100例的临床资料,按随机数字表法分为观察组50例和对照组50例。观察组给予交锁髓内治疗,对照组给予常规钢板内固定治疗。比较两组的手术各项指标、临床疗效。结果观察组术中出血量、手术时间、骨折愈合时间、术后感染情况分别为(265.31±10.35)m L、(98.25±8.24)min、(17.25±1.51)min、2例,对照组为(285.37±11.54)m L、(101.87±6.57)min、(19.51±2.02)min、8例,观察组均优于对照组,差异有统计学意义(P<0.05)。观察组临床有效率为98.00%,高于对照组的68.00%,差异有统计学意义(P<0.05)。结论交锁髓内钉治疗四肢骨干严重粉碎性骨折临床疗效显著,具有更高优势,能够减少患者术中出血量,愈合率效果,促进病情恢复。

Objective To explore the clinical value of interlocking intramedultary nail in treatment of severe comminuted fracture of extremities. Methods Clinical data of 100 cases of severe comminuted fractures of extremities cured in our hospital from Anguest 2013 to November 2015 were trtrospectively analyzed.According to random digital table method,they were divided into observation group and control group with 50 cases in each.Patients in observation group were treated with interlocking intramedullary nailing,and patients in control group were treated with conventional plate internal fixation.Operation index and clinical efficacy of the two groups were compared. Results Intraoperative blood loss,operation time,fracture healing time,postoperative infection of observation group were （265.31 ± 10.35）mL,（98.25 ± 8.24min,（17.25 ± 1.51）min and two cases,which were better than those of control group with（285.37 ± 11.54）mL,（101.87 ± 6.57）min,（19.51 ± 2.02）min and 8 eases,and the difference was statistical significance（P 〈 0.05）. Conclusion The effect of interlocking intramedullary nail in treatment of severe comroinuted fracture of extremities is significant.h has a higher advantage.It can reduce the amount of intraoperative bleeding. Healing rate effect is good.It promotes disease recovery.
