摘要
目的探讨和分析耳后注射甲泼尼龙琥珀酸钠和利多卡因注射液在急性耳鸣患者治疗中的临床效果。方法对临床就诊的急性耳鸣患者76例,随机分为治疗组38例(38耳),对照组38例(38耳)。所有患者在治疗前、治疗结束时均分别行纯音测听、相关检查和国际通用的耳鸣致残量表(THI)进行评分,得分均在20分以上。其中治疗组38例(38耳)耳后注射甲泼尼龙琥珀酸钠40mg+利多卡因注射用液0.2m L,隔2日1次,连续用药5次,同时静滴或口服银杏叶制剂或中成药,并辅以心理疏导。对照组除不行耳后注射外,其余治疗同治疗组。比较两组治疗效果。结果对照组有效8例(21.1%),治疗组有效22例(57.9%),治疗组有效率优于对照组;治疗组THI评分明显低于对照组(P<0.05)。结论耳后注射甲泼尼龙琥珀酸钠+利多卡因注射液能改善急性耳鸣,是一种安全有效的治疗方法,是全身应用激素的替代疗法。
Objective To investigate and analyze the clinical effection of methotrexate sodium succinate and lidocaine injection in the treatment of acute tinnitus patients. Methods 76 patients with acute tinnitus were randomly divided into 2 groups:treatment group (38 ears) and control group (38 ears). Before and after the treatment, all patients were tested with pure tone audiometry, related tests and international general tinnitus disability scale (THI). All the scores were more than 20 points. 38 cases (38 ears) in treatment group were treated respectively with g0mg retroauricular injection of methylprednisolone sodium succinate and 0.2ml lidocaine liquid, 1 time per three days, total injections for 5 times, and with intravenous or oral Ginkgo biloba preparations or proprietary Chinese medicines, supplemented with psychological counseling. Patients in control group were treated with intravenous or oral Ginkgo biloba preparations or proprietary Chinese medicines, supplemented by psychological counseling. Results 8 cases (21.1%) of the control group were effectively cured, while 22 cases (57.9%) of the treatment group were effectively cured. The effectiveness of treatment group was better than that of control group. THI score in treatment group was obviously lower than that in control group (P 〈 0.05). Conclusion The study shows that retroauricular injection of methylprednisolone sodium succinate and lidocaine injection is a safe and effective treatment in acute tinnitus. It is a replacement theranv for svstemic application of hormone.
