晚期乳腺癌终末期的中医人文关怀护理

TCM humanistic care nursing of patients with advanced breast cancer in end stage

[目的]研究中医人文关怀对晚期乳腺癌终末期病人生活质量的影响及其效果。[方法]回顾性分析湖南中医药大学第一附属医院乳腺科及肿瘤科2014年3月—2016年3月治疗的41例晚期乳腺癌终末期病人,总结其终末期特点、医治及护理过程中遇到的问题,并随机分为关怀组及对照组,对照组给予常规护理及治疗,关怀组给予基础护理、疼痛护理、心理护理等终末期人文关怀措施。[结果]15例本病终末期病人存在脏器感染;25例本病终末期病人发生骨转移,临床症状以"骨痛"为主;13例本病终末期病人存在各脏器转移;36例本病终末期病人存在低蛋白血症及血红蛋白减少;在心理状态、疼痛缓解以及医疗满意度等指标,关怀组明显优于对照组(P<0.05)。[结论]晚期乳腺癌终末期病人实施人文关怀可提高病人的生活质量,并延长其生存期,亦可缓解医患之间关系。

Objective：Through the analysis of medical cases and nursing records and concrete methods of humanistic care of advanced breast cancer patients with end-stage in first affiliated hospital of Hunan University of Chinese Medicine,and study the influence of humanistic care on the quality of life of patients with end-stage breast cancer and its effect,summarize its methods and provide the basis for the dying humanistic care theory in breast cancer patients with end-stage.Methods：A retrospective analysis was carried out for 41 cases of advanced breast cancer patients from breast department and cancer department in first affiliated hospital of Hunan University of Chinese Medicine from March 2014 to March 2016.It summed up the characteristics of end-stage,the problems in treatment and nursing process,and these patients were randomly divided into concern group and control group.These patients in control group were given routine nursing and treatment,the patients in care group were given the basic nursing,pain nursing,psychological nursing and other effective end-stage humanistic care measures.Results：There were 15 cases of advanced breast cancer patients with end-stage had organ infection;25cases of patients with end-stage had bone metastases,clinical symptom was given priority to＂bone pain＂;13cases of patients with end-stage had viscera metastasis;36cases of patients with end-stage had hypoalbuminemia and hemoglobin reduced.The psychological state,pain relief and health satisfaction indexes in care group were better than those in control group（P0.05）.Conclusions：The focus of humanistic care is to observe and evaluate the patient＇s condition changes,to carry out detailed medical care and nursing care of their physiology,to ease and appease their psychological.It explained their family work and health guidance,in general,can improve the late breast,quality of life of patients with end-stage cancer,and extend their survival,but also ease the relationship between doctors and patients.
