清肝祛瘀转阴汤联合苦参碱治疗慢性乙型肝炎随机平行对照研究

Randomized Controlled Trial of Qinggan Quyu Zhuanyin Decoction(清肝祛瘀转阴汤) Combined with Matrine in the Treatment of Chronic Hepatitis B

[目的]观察清肝祛瘀转阴汤联合苦参碱治疗慢性乙型肝炎疗效。[方法]使用随机平行对照方法,将130例门诊患者按抛硬币方法简单随机分为两组。对照组42例苦参碱0.15g+10%葡萄糖250m L,1次/d,静滴。治疗组78例清肝祛瘀转阴汤,水煎400m L,1剂/d,早晚2分服。苦参碱粉针;治疗同对照组。连续治疗6个月为1疗程。观测临床症状、ALT、AST、TBi L、HBs Ag;Hbe Ag、HBV-DNA、不良反应。治疗1疗程(6个月),判定疗效。[结果]治疗组显效33例,有效36,无效9例,总有效率88.50%;对照组显效11例,有效20例,无效11例,总有效率73.80%;治疗组疗效优于对照组(P<0.05)。ALT、AST、TBi L两组均有改善(P<0.01,P<0.05),治疗组改善优于对照组(P<0.01)。血清HBs Ag、HBe Ag转阴率两组无显著差异(P>0.05);HBV-DNA转阴率治疗组高于对照组(P<0.01)。[结论]清肝祛瘀转阴汤联合苦参碱治疗慢性乙型肝炎,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the linggan Quyu Zhuanyin decoction combined with matrine in the treatment of chronic hepatitis b curative effect. [Method] 130 patients were randomly divided into two groups according to the coin tossing method. The control group of 42 cases of matrine injection 0.15g＋10% glucose 250mL, 1time/d, intravenous drip. The treatment group of 78 cases of liver blood stasis Zhuanyin decoction, 400mL, 1 doses of/d, sooner or later 2 hours service. Matrine injection; treatment with the control group. Continuous treatment for 6 months for a course of 1. Clinical symptoms, ALT, AST, TBiL, HBsAg, HbeAg, HBV-DNA, adverse reactions were observed. Treatment 1 course of treatment （for 6 months）, judge curative effect. [Result] In the treatment group, 33 cases were markedly effective, effective in 36 cases, ineffective in all cases, the total effective rate was 88.50%, the control group was markedly effective in 11 cases, effective in 20 cases, ineffective in 11 cases, the total effective rate was 73.80%; the treatment group was better than the control group （P〈0.05）. ALT, AST, TBiL two groups were improved （P〈0.01, P〈0.05）, the treatment group was better than the control group （P〈0.01）. Serum HBsAg, HBeAg negative rate of two groups had no significant difference （P〉0.05）; HBV-DNA negative rate higher than the treatment group （P〈0.01）. [Conclusion] inggan Quyu Zhuanyin decoction combined with matrine in the treatment of chronic hepatitis B, the curative effect is satisfied, no serious adverse reactions, worthy of promotion.
