独活寄生汤对膝骨性关节炎患者关节液炎症因子、疗效及膝关节功能状况的影响分析

Effect of Duhuo Jisheng Decoction on Knee Osteoarthritis Patients' Synovial Fluid Inflammatory Factors and Curative Effect of Knee Function Status

目的:分析独活寄生汤对膝骨性关节炎患者的关节液炎性因子、疗效和膝关节功能状况的影响。方法:选择2014年1月—2016年1月期间60例来医院就诊的膝骨性关节炎患者进行本项研究。按照随机数表法,将60例患者平均分为观察组和对照组两组。对照组给予双氯芬酸钠缓释片口服治疗;观察组在对照组西药口服治疗的基础上,加用独活寄生汤治疗。两组患者治疗4周后,分析两组患者TNF-α、IL-6和MMP-1治疗前后水平变化情况,比较两组患者临床治疗疗效。采用Lysholm-II量表评价患者治疗前后膝关节功能变化。结果:两组患者的TNF-α、IL-6和MMP-1三者水平同本组治疗前相比明显下降,差异明显(P<0.05)。治疗前,两组患者的TNF-α、IL-6和MMP-1水平差异不明显(P>0.05)。治疗后5周,观察组和对照组的TNF-α、IL-6和MMP-1水平差异具有统计学意义(P<0.05)。观察组患者临床治疗总有效率为93.33%,明显高于对照组治疗总有效率(73.33%),两组患者临床治疗疗效差异具有统计学意义(P<0.05)。与治疗前相比,治疗后两组患者的Lysholm-II评分均有所提高。治疗前两组患者的Lysholm-II评分差异不明显(P>0.05)。治疗1、3、5周后,两组患者的Lysholm-II评分差异具有统计学意义(P<0.05)。结论:独活寄生汤能显著降低膝骨性关节炎患者的关节液炎症因子TNF-α、IL-6和MMP-1水平,提高患者的临床治疗疗效,显著改善患者的膝关节功能状况,值得在临床推广应用。

Objective： To analyze the effect of Duhuo Jisheng Decoction on knee osteoarthritis patients＇ synovial fluid inflammatory factors,the effect and function of the knee. Methods： Between January 2014 and January 2016,60 patients with knee osteoarthritis in our hospital were selected for this study. According to the random number table method,they were randomly divided into observation group and control group. The control group was given Diclofenac Sodium Sustained Release Tablets. The observation group combined Duhuo Jisheng Decoction. After 4 weeks of treatment,two groups＇ TNF-α,IL-6 and MMP-1 levels were analyzed before and after treatment and the clinical efficacies of the two groups were compared. Evaluate knee function in patients before and after treatment with Lysholm-Ⅱ scale. Results： The levels of TNF-α,IL-6 and MMP-1 in the two groups were significantly decreased compared with those before treatment and the difference was significant（ P〈0. 05）. Before treatment,the two groups＇ TNF-α,IL-6 and MMP-1 levels were not significantly different（ P〈0. 05）. After 5 weeks of treatment,the levels of MMP-1,IL-6 and TNF-α in the observation group and the control group were significantly different（ P〈0. 05）. The total effective rate of observation group was 93. 33%,which was significantly higher than that of control group（ 73. 33%） and the difference of therapeutic effect between the two groups was statistically significant（ P〈0. 05）. Compared with those before treatment,the Lysholm-Ⅱ scores of the two groups after treatment were improved. Before treatment,the difference of Lysholm-Ⅱ score between the two groups was not significant（ P〉0. 05）. After 1,3 and 5 weeks,the difference of Lysholm-Ⅱ score between the two groups was statistically significant（ P〈0. 05）. Conclusion： Duhuo Jisheng Decoction can significantly reduce the knee osteoarthritis patients＇ synovial fluid inflammatory factor TNF-α,IL-6 and MMP-1 levels,improve the clinical curative effect of patients and significantly improve knee function in patients. It is worthy of clinical application.
