老年COPD合并呼吸衰竭的临床表现分析及研究

Analysis and Study of Clinical Features of Elderly COPD Patients Combined Respiratory Failure

目的:探索老年COPD合并呼吸衰竭的临床表现。方法:将150例老年COPD患者研究对象分为3组,COPD急性加重期组、COPD急性加重期合并心衰组和COPD缓解期组,每组50例患者,检测所有患者甲状腺激素水平。结果:COPD急性加重期合并心衰组患者TSH、FT_4、TT_4、FT_3、TT_3水平明显低于COPD急性加重期组和COPD缓解期组,同时死亡组TSH、FT_4、TT_4、FT_3、TT_3水平明显低于生存组(P<0.05)。结论:老年COPD合并呼吸衰竭患者主要表现为甲状腺激素水平下降,通过早期检测患者甲状腺激素水平,可防止患者病情加重。

Objective： To explore the clinical features of elderly COPD patients with respiratory failure. Methods. 150 cases of elderly COPD patients were divided into three groups including a group of acute exacerbation of COPD patients, a group of acute exacerbation of COPD patients with respiratory failure, and a group of COPD remission patients,with 50 cases in each group,then the thyroid hormone levels of all patients were detected.Results： The TSH, FT4, TT4 ,FT3 ,TT3 levels of group of acute exacerbation of COPD patients with respiratory failure were significantly lower than those of group of acute exacerbation of COPD patients and group of COPD remission patients, and the TSH, FT4 , TT4, FT3 , TT3 levels of death group were significantly lower than those of survival group （P〈0.05）.Conclusion. The main manifestations of elderly COPD patients with respiratory failure was the decrease of thyroid hormone levels, so through early detection of patients＇ thyroid hormone levels can prevent patients＇ condition.
