摘要
目的:分析胎儿蛛网膜囊肿的产前超声影像学特征,并探讨其临床诊断价值。方法:选取某院2012年1月~2015年12月间进行产前超声检查的孕产妇18936例的临床资料,总结胎儿蛛网膜囊肿的超声影像学特征。结果:超声检测准确率高达100%,高于MRI检测的86.7%,差异具有统计学意义(χ~2=9.084,P<0.05);超声检测影像学特征是:胎儿颅脑出现不规则或圆形的无回声区,囊壁较为光滑并且很薄,囊肿多位于脑裂缝的间隙出或脑表面处,且与侧脑之间互不连通,超声血流显像显示囊肿内无血流信号。结论:胎儿蛛网膜囊肿采用超声检测准确性高于MRI检测,对于单个较小的蛛网膜囊肿,宜进行定期随访,若囊肿没有增大或发生其他异常情况,可继续妊娠,反之若发现合并其他畸形、囊肿逐渐变大或染色体检查阳性等情况下宜考虑终止妊娠。
Objective:To analyze imaging features and diagnostic value of Prenatal ultrasound in the detection of Fetal arachnoid cyst.Methods: The clinical data of 18,936 cases of pregnant women with prenatal ultrasound examination in a hospital from January 2012 to December 2015 were selected and the imaging features of fetal arachnoid cyst were summarized.Results: The ultrasonic detection accuracy (100%) was higher than that of MRI examination (86.7%),which tthe difference was statistically significant (x2=9.084, P〈 0.05) ;The imaging features were including that there was irregular or circular anechoic region in the fetal brain,the bladder wall was smooth and thin,the cyst was located in the gap of brain fissure or the surface of brain and is not connected with the lateral brain, and ultrasound imaging showed no blood flow signal in the cyst.Conclusion:The detection accuracy of ultrasound for fetal arachnoid cyst is higher than that of MRI detection.For a single small arachnoid cyst,regular follow-up should be conducted,if the cyst is not increased or there was no other abnormalities, the pregnancy can be continued, on the other hand, if the combined other malformations are found, cyst becomes larger, or chromosome is positive, the termination of pregnancy should be considered.
