无创呼吸机在COPD合并呼吸衰竭患者中的应用效果分析

Application Effect of Noninvasive Ventilator in COPD Patients with Respiratory Failure

目的:评价无创呼吸机在COPD合并Ⅱ呼吸衰竭患者中的应用效果,探讨其临床适用性。方法:选择从2014年3月~2015年12月于某院就诊的39例COPD合并Ⅱ呼吸衰竭患者,所有患者均给予低流量吸氧、抗感染、化痰止咳等基础对症治疗,同时给予BiPAP无创呼吸机辅助通气治疗,观察患者治疗前后肺功能检查及动脉血气指标改善情况。结果:患者经治疗后FRC/FVC/FEV1%肺功能指标均较治疗前显著改善(P<0.05),患者治疗后各项动脉血气指标均较治疗前明显改善(P<0.05)。结论:无创呼吸机联合常规治疗在COPD合并Ⅱ呼吸衰竭中有很好的治疗效果,可以显著改善患者的肺功能及纠正呼吸衰竭,适合临床长期推广应用。

Objective：To evaluate the clinical application effect of noninvasive ventilator in COPD patients with respiratory failure,and to explore its clinical applicability.Methods： 39 cases of COPD patients with type II respiratory failure treated in a hospital from March 2014 to December 2015 were selected,and all patients were given low flow oxygen, anti infection, preventing phlegm from forming and stopping coughing, and other primary symptomatic treatment,also given BiPAP non-invasive ventilation treatment,then then improvement of pulmonary function and arterial blood gas indexes before and after treatment were observed.Results：After treatment,the pulmonary function indexes of FRC/FVC/FEV1 % were significantly improved （P 〈 0.05）, and the arterial blood gas indexes were significantly improved after treatment （P 〈0.05）.Conclusion： Noninvasive ventilator combined conventional treatment in the treatment of COPD patients with respiratory failure has a good effect, which can significantly improve patients＇ pulmonary function and correct respiratory failure, and is suitable for clinical long-term promotion and application.
