摘要
目的:对比分析超声弹性成像与灰阶超声检查在甲状腺结节良恶性鉴别诊断中的应用价值。方法:选取2014年5月~2016年3月某院收治的甲状腺单发结节患者85例为研究对象,均接受手术治疗,患者术前分别接受灰阶超声与超声弹性成像检查。以术后病理诊断为金标准,观察灰阶超声与超声弹性成像对甲状腺结节诊断的准确性、特异性及敏感性。结果:术后病理诊断结果显示:85例患者中,恶性结节患者32例(37.65%),良性结节患者53例(62.35%),灰阶超声检查结果显示:85例患者中,恶性结节患者42例(49.41%),良性结节43例(50.59%);超声弹性成像检查结果显示:85例患者中,恶性结节患者50例(58.82%),良性结节35例(41.18%)。超声弹性成像诊断准确性、特异性、灵敏性分别为94.34%、96.88%、92.45%,灰阶超声诊断准确性、特异性、灵敏性分别为79.25%、81.25%、67.92%,差异均有统计学意义(P<0.05)。结论:在甲状腺结节良恶性鉴别诊断中,采用超声弹性成像检查具有较高准确性、特异性及敏感性,可为治疗方案制定提供可靠指导,值得推广。
Objective: To analyze the application value of ultrasound elastography and gray-scale ultrasonography in the diagnosis of benign and malignant thyroid nodules.Methods.85 cases of patients with solitary thyroid nodules treated in a hospital from May 2014 to March 2016 were selected as research objects.All patients were given surgical treatment, and they received gray scale ultrasound and ultrasound elastography examination before surgery. The postoperative pathological diagnosis was taken as the gold standard to observe the accuracy, specificity and sensitivity of gray-scale ultrasonography and ultrasound elastography in the diagnosis of thyroid nodules. Results-The results of postoperative pathological diagnosis showed that among 85 cases of patients there were 32 cases of patients with malignant nodules (37.65%) and 53 cases of benign nodules (62.35 %).The results of gray-scale ultrasonography showed that among 85 cases of patients there were 42 cases of patients with malignant nodules (49.41%) and 43 cases of benign nodules (50.59%).The results of ultrasound elastography showed that among 85 cases of patients there were 50 eases of malignant nodules (58.82 %) and 35 cases of benign nodules (41.18 %). The diagnostic accuracy, specificity, and sensitivity of ultrasonic elastography were 94.34 %, 96.88 %, 92.45 % respectively, and those of gray-scale ultrasonography were 79.25%,81.25%,67.92% respectively,which the differences were statistically significant (P〈 0.05).Conclusion: Ultrasound elastography in the diagnosis of benign and malignant thyroid nodules has high accuracy, specificity and sensitivity, which can provide reliable guidance for the development of treatment programs, and is worth promotion.
