儿童过敏性紫癜复发相关临床因素分析

Clinical factors associated with recurrence of Henoch-Schonlein purpura in children

目的:探讨儿童过敏性紫癜(HSP)复发的相关临床因素。方法:回顾性分析2010年1月至2013年12月江门市中心医院收治的118例HSP患儿的临床资料,随访1年后将患者分为无复发组和复发组,分析两组发病年龄、性别、前驱感染、过敏原阳性、Ig A升高、重症病例、1周内发现尿常规异常、开始治疗时间及敏性紫癜的发病类型等临床指标的差异。结果:过敏性紫癜复发率为29.66%,复发时间均在治疗后6个月内,前驱感染、开始治疗时间及过敏性紫癜的发病类型与复发无关(P>0.05);而两组发病年龄、性别、过敏原阳性、Ig A升高、重症病例及1周内发现尿常规异常比较,差异有统计学意义(P<0.05),其中重症病例复发率为100%。结论:发病年龄、性别、过敏原阳性、Ig A升高、重症病例及1周内发现尿常规异常与HSP复发关系密切,特别是重症病例,在初诊和随访过程中应提高警惕,密切观察。

Objective： To investigate the clinical factors associated with the recurrence of Henoch-Schonlein purpura（ HSP） in children. Methods： The clinical data of 118 children with HSP,who were hospitalized in the Jiangmen Municipal Central Hospital between January 2010 and December 2013,were retrospectively analyzed. At1 year after follow-up,the patients were divided into the recurrence-free group and the recurrence group. The differences in the age of onset,sex,prodromic infection,allergen-positive,increased Ig A,severe cases,abnormalities in routine urine test at 1 week,onset of treatment and types of HSP between the two groups were analyzed. Results： The recurrence rate of HSP was 29. 66%,and the recurrence occurred at 6 months after the treatment. There was no correlation between the prodromic infection,onset of treatment,types of HSP and the recurrence（ P〉0. 05）. There were statistically significant differences in the age of onset,sex,allergen-positive,increased Ig A,severe cases,and abnormalities in routine urine test at 1 week between the two groups（ P〈0.05）.The recurrence rate of severe cases was 100%. Conclusion： The age of onset,sex,prodromic infection,allergenpositive,increased Ig A,severe cases,abnormalities in routine urine test at 1 week,and onset of treatment are closely related to HSP recurrence,especially in severe cases,which should be highly alerted and determined closely during the primary diagnosis and follow-up.
