中医体质调摄对社区老年糖尿病患者治疗效果与生存质量的影响

Effect of TCM Constitution Regulation on Treatment Effect and Life Quality of Elder Patients with Type 2 Diabetes Mellitus

目的:探讨中医体质调摄对社区65岁以上老年2型糖尿病(T2DM)患者治疗效果与生存质量的影响。方法:选取我中心65岁以上老年T2DM患者218例,随机分为观察组112例和对照组106例,对照组接受常规健康管理,观察组在对照组的基础上,由专人进行中医体质辨识,并定期给予中医体质调摄,中医体质调摄内容包括饮食调养、运动保健、穴位保健、情志与起居调摄等。比较2组患者干预前后糖尿病(DM)生存质量特异性量表(DSQL)评分与DM代谢控制指标的变化。结果:观察组112例老年T2DM患者大多数都属于偏颇体质,以痰湿质、气虚质居多,分别占老年T2DM患者的29.5%、18.8%。经过1年的干预,观察组患者对T2DM引起的生理、心理、社会和治疗等4个维度的DSQL评分明显低于对照组,差异有统计学意义(P<0.05),观察组患者的体质指数、空腹血糖、胆固醇、三酰甘油等临床指标均值明显低于对照组,差异有统计学意义(P<0.05)。结论:实施中医体质调摄,能显著改善社区65岁以上老年T2DM患者的治疗效果与生存质量。

Objective：To explore the influence of Traditional Chinese Medicine（ TCM） constitution regulation on treatment effect and life quality of patients aged over 65 years with type 2 diabetes mellitus（ T2DM） in community patients.Methods：A total of218 T2 DM patients aged 65 years and above were recruited and randomly divided into observation group（ 112 cases） and control group（ 106 cases）.Both groups were treated with routine health management,while the observation group received additional TCM constitution regulation with exact guidance on diet,sports,acu-point press,living habits,and emotion regulation.Comparisons were made on the changes of diabetes specific quality of life scale（ DSQL） scores,body mass index（ BMI）,fasting blood glucose（ FPG）,cholesterol,triglycerides and other clinical indicators.Results：Most of the T2 DM patients in observation group were retroactively classified as biased body constitution.To be specific,phlegm-dampness constitution（ 29.5%） and qi deficiency constitution（ 18.8%） account for the highest proportion.After 1 year of intervention,4 DSQL scores including＂physical health＂,＂mental health＂,＂social capability＂and＂treatment effect＂of the observation group were significantly lower than those of the control group（ P〈0.05）.Furthermore,compared with the control group,the values of BMI,FPG,cholesterol and triglycerides in observation group were decreased with statistical difference compared with the control group（ P〈0.05）.Conclusion：TCM constitution regulation can significantly improve the treatment effect and life quality of T2 DM patients aged 65 years and above in the community.
