布地奈德联合不同药物雾化吸入治疗儿童哮喘疗效比较

Comparison of clinical efficacy of budesonide inhalation combined with different medicines against pediatric asthma

目的:比较布地奈德联合特布他林,布地奈德联合特布他林及异丙托溴铵雾化吸入治疗儿童哮喘的临床疗效。方法:将某院急性发作期中度哮喘患儿74例,随机分为对照组和治疗组。在吸氧、止咳祛痰等常规治疗基础上,对照组给予布地奈德联合特布他林雾化吸入,治疗组在以上治疗的基础上加用异丙托溴铵雾化吸入。按体质量给予不同剂量药物,7 d为1疗程。分析比较2组患儿的临床疗效、临床症状消失时间、肺功能及实验室指标的变化。结果:治疗组患儿的总有效率明显高于对照组,治疗组的临床症状消失时间和住院时间明显比对照组短,2组患儿在总有效率、临床症状消失时间和住院时间上差异存在显著性(P<0.05);治疗后,2组患儿的肺通气功能指标均有明显上升,治疗组患儿的肺通气功能指标显著高于对照组,组间差异有统计学意义(P<0.05);治疗后,除大于20 kg患儿的EOS%指标外,治疗组患儿的其他实验室指标均明显低于对照组(P<0.05)。结论:雾化吸入布地奈德联合特布他林及异丙托溴铵的疗效优于布地奈德联合特布他林,症状消失时间缩短、肺功能明显改善并能够减少不良反应是治疗儿童哮喘的有效选择。

OBJECTIVE To compare the clinical efficacy of budesonide inhalation combined with terbutaline and budesonide inhalation combined with terbutaline and ipratropium bromide against pediatric asthma.METHODS Totally 74 patients with moderate asthma in acute episode were randomly divided into control group and observation group.Based on routine treatments such as oxygen therapy and cough expectorant,patients in control group were given budesonide and terbutaline,while patients in observation group received ipratropium bromide based on basic treatment of control group.Different doses were given based on weight,and seven days was set as a course of treatment.Clinical efficacy,disappearance time of symtpoms,lung functions and changes in laboratory indexes were analyzed and compared.RESULTS The total effective rate of observation group was higher than that of control group,disappearance time of symptoms and hospital stay in observation group were shorter than those in control group.Patients in the 2 groups had significantly difference in total effective rate,disappearance time of symptoms and hospital stay（P〈0.05）.After treatment,lung function index in both groups were improved obviously,the lung function index of observation group were considerably higher than those of control group（P〈0.05）.After treatment,except EOS%index in pediatric patients over 20 kg,other laboratory indexes of the observation group were lower than those of control group（P〈0.05）.CONCLUSION The efficacy of budesonide inhalation combined with terbutaline and ipratropium bromide is superior to that of budesonide inhalation combined with terbutaline,as it can shorten disappearance time of symptoms and improve lung functions with less adverse reactions.Therefore,it is an effective choice for the treatment of pediatric asthma.
