儿童肠系膜淋巴结炎72例诊治分析

Diagnosis and treatment of 72 children with mesenteric lymphadenitis

目的探讨儿童肠系膜淋巴结炎的临床表现、诊断和治疗方法，提高诊治水平。方法对2015年5月至2016年7月本院诊治的72例肠系膜淋巴结炎患儿的临床资料进行回顾性分析，分析其临床特点、辅助检查和治疗方法。结果72例肠系膜淋巴结炎患儿均有腹痛症状，其中脐周压痛49例（68．06％），右下腹压痛19例（26．39％）。高频超声显示腹腔淋巴结大小为12．5mm×6．0mm-32．0mm×15．0mm，直径〉12．0mm，横径〉6．0mm，纵横径比值（L／S）〉2；它能有效辅助诊断和鉴别肠系膜淋巴结炎。应用清热散结片联合思连康片治疗后显效40例（55．56％），有效32例（44．44％），总有效率为100．00％。结论肠系膜淋巴结炎患儿均有腹痛症状，高频超声是诊断鉴别儿童肠系膜淋巴结炎的重要手段，清热散结片和思连康片联合治疗儿童肠系膜淋巴结炎效果良好。

Objective To investigate the clinical manifestation, diagnosis, and treatment of mesenteric lymphadenitis in children and improve the diagnosis and treatment. Methods The clinical data of 72 children with mesenteric lymphadenitis treated at our hospital from May, 2015 to July, 2016 were retrospectively analyzed. Their clinical features, auxiliary examination, and treatment were analyzed. Results All the 72 children had abdominal pain; among which, 49 cases （68.06%） got navel tenderness and 19 cases （26.39%） right lower quadrant tenderness. The size of the peritoneal lymph nodes was 12.5 mmx 6.0 mm to 32.0 mm x 15.0 mm; the diameter was 12.0 mm; the transverse diameter was over 6.0 mm; and the ratio of longitudinal and transverse diameter （L / S）〉 2. High-frequency ultrasonography could effectively assist in the diagnosis and identification of mesenteric lymphadenitis. After the application Qingresanjie tablets and Siliankang tablets, 40 cases （55.56%） were markedly effective and 32 cases （44.44%） effective and the total effective rate was 100.00%. Conclusions Children with mesenteric lymphadenitis have abdominal pain symptoms. High-frequency ultrasonography is an important method for the diagnosis and differential diagnosis of mesenteric lymphadenitis in children. The combination of Qingresanjie tablets and Siliankang tablets for mesenteric lymphadenitis in children is effective.
