延续性护理服务模式对综合性医院老年失能出院患者的影响分析

Analysis of the Effect of Continuous Nursing Service Model on the Elderly Patients With Disability Discharge

目的研究在综合性医院老年失能出院患者中实施延续性护理的临床效果。方法选取我院2014年2月—2015年2月诊治的260例老年失能出院患者,并对所有资料实施回顾性分析,分析实施延续性护理后老年失能出院患者的临床效果。结果在对老年失能患者实施延续性护理之后并发症发生率以及再入院率显著降低,差异具有统计学意义(P<0.05)。结论综合性医院将延续性护理干预应用在老年失能出院患者中效果显著。

Objective To study the elderly in general hospital of disability in patients discharged from the implementation of the clinical effect of continuing nursing care. Methods 260 cases of elderly patients in our hospital during the period from February 2014 to February 2015 in the diagnosis and treatment of disabled patients, and carry out the retrospective analysis of all data, analysis of the implementation of continuing nursing care after the elderly disabled patients discharged with clinical effect. Results In the elderly disabled patients after continuous nursing complications and readmission rate decreased significantly, statistical significance （P〈0.05）. Conclusion The comprehensive hospital will continue nursing intervention applied in elderly disabled patients has significant effect.
