腹腔镜手术、开腹手术在异位妊娠治疗中的应用效果

The Effect of Laparoscopic Surgery and Laparotomy in the Treatment of Ectopic Pregnancy

目的研究并比较腹腔镜手术、开腹手术在异位妊娠治疗中的应用效果。方法选取2013年1月—2016年10月我院产科收治的100例异位妊娠患者作为研究对象,将其随机分为对照组与观察组,每组各50例。对照组、观察组分别采取开腹手术、腹腔镜手术治疗,比较两组手术治疗效果。结果观察组的手术用时与对照组比较,差异无统计学意义(P>0.05)。观察组术中失血量、首次排气时间、住院时间均少于对照组,差异具有统计学意义(P<0.05);观察组患者疼痛评分低于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05);其输卵管再通率与对照组相比,差异无统计学意义(P>0.05)。结论对异位妊娠患者实施腹腔镜手术、开腹手术均可促使输卵管恢复通畅,但腹腔镜手术可减少手术创伤,减轻术后疼痛,有利于加快术后恢复,可作为异位妊娠的首选术式。

Objective To study and compare the effect of laparoscopic surgery and laparotomy in the treatment of ectopic pregnancy. Methods 100 cases of ectopic pregnancy treated in our hospital from January 2013 to October 2016 were selected as the research object and were randomly divided into control group and observation group, 50 cases in each group. The control group and the observation group were treated with open surgery and laparoscopic surgery. The effects of two groups were compared. Results Compared with the control group, the operation time of the observation group was compared with that of the control group, there was no significant difference （P〉0.05）. The blood loss, the first exhaust time and hospitalization time in the observation group were less than those in the control group, and the difference was statistically significant （P〈0.05）. The pain score of the observation group was lower than that of the control group （P〈0.05）. The complication rate of the observation group was lower than that of the control group （P〈0.05）. Compared with the control group, the recanalization rate of fallopian tube was not statistically significant （P〉0.05）. Conclusion Laparoscopic surgery and open surgery for ectopic pregnancy can protnote the recovery of fallopian tube patency, laparoscopic surgery can reduce surgical trauma, reduce postoperative pain. It is helpful to accelerate postoperative recovery, which can be used as the first choice for ectopic pregnancy.
