针刺“咽四穴”联合构音训练治疗脑卒中后痉挛型构音障碍的临床观察

The Application of Acupuncture at Yansi Point and Articulation Training in the Treatment of Patients with Spastic Dysarthria after Stroke

目的:观察针刺"咽四穴"联合构音训练治疗脑卒中后痉挛型构音障碍的疗效。方法:将脑卒中后痉挛性构音障碍患者随机分为试验组30例和对照组30例,试验组采用针刺"咽四穴"联合构音训练治疗,对照组单纯采用构音训练治疗。采用NIHSS评分及改良Frenchay构音障碍等级评定法评定两组治疗情况。结果:治疗后,两组患者的NIHSS评分均较治疗前显著降低(P<0.05),且试验组治疗后的NIHSS评分显著低于对照组(P<0.05);治疗后,两组患者的Frenchay构音障碍等级均较治疗前显著改善(P<0.01),且试验组治疗后的Frenchay构音障碍等级改善显著优于对照组(P<0.01)。结论:针刺"咽四穴"联合构音训练治疗脑卒中后痉挛型构音障碍有良好的临床效果。

Objective; To observe the effect of acupuncture at Yansi point and articulation training in the treatment of patients with spastic dysarthria after stroke. Methods; 60 patients with spastic dysarthria after stroke were randomly divided into two groups with 30 patients in each group. The treatment group received the articulation training and acupuncture at Yansi point, while the control group only received the articulation training. Frenchay dysarthria level was evaluated before and after the treatment. Results; The two groups before treatment NIHSS score difference has no statistical significance （p〉0.05）, after treatment the NIHSS scores was lower （p〈0.05）. The NIHSS score of the experimental group was significantly lower than that of the control group. Two groups of patients with Frenchay dysarthria levels Were significantly improved compared with before treatment（p〈0.01）. Frenchay dysarthria level of the treatment group were significantly higher than the control group after the treatment （p〈0.01）. Conclusion; The combined use of acupuncture at Yansi point and articulation training is effective in treating patients with spastic dysarthria after stroke, and is significantly better than the articulation training only in improving the speech communication abilities of the patients.
