腹腔镜与开腹手术下行胃穿孔修补术的综合疗效对比研究

Comparative study of the comprehensive efficacy of laparoscopic and open surgery on gastric perforation repair

目的对比腹腔镜与开腹手术下行胃穿孔修补术的综合疗效。方法选取我院2014年2月~2016年2月胃穿孔患者40例,分两组,每组20例。开腹组开腹手术下行胃穿孔修补术,腹腔镜组腹腔镜手术下行胃穿孔修补术。比较两组患者的术后消化道功能恢复正常的时间、术后机体康复天数、修补术过程平均出血量、术后平均引流量和胃穿孔修补术后并发症发生率,及两组的手术综合疗效。结果腹腔镜组手术综合疗效明显高于开腹组,术后并发症发生率明显低于开腹组(P<0.05)。腹腔镜组患者术后消化道功能恢复正常的时间、术后机体康复天数、修补术过程平均出血量、术后平均引流量均明显优于开腹组(P<0.05)。结论腹腔镜与开腹手术下行胃穿孔修补术均有一定的效果,但腹腔镜下行胃穿孔修补术效果更好,创伤更小,修补中出血少,术后引流量减少,且消化道功能和机体功能恢复快,并发症少,值得推广。

Objective To investigate and compare the comprehensive efficacy of laparoscopic and open surgery for gas- tric perforation repair. Methods 40 patients with gastric perforation from February 2014 to February 2016 were select- ed in our hospital and assigned to two groups, with 20 patients in each group. Open surgery group was given gastric perforation repair under laparotomy,laparoscopy group was given gastric perforation repair under laparoscopy. The time of postoperative gastrointestinal function recovery, the average number of days of physical recovery after surgery, the average bleeding volume during the repair process, the average postoperative drainage volume and the incidence rate of postoperative complications of gastric perforation repair, and the comprehensive effect of surgery were compared be- tween the two groups. Results The comprehensive efficacy of laparoseopic surgery was significantly higher than that in the open surgery group, and the incidence rate of postoperative complications of gastric perforation repair was signifi- cantly lower than that in the open surgery group （P〈0.05）. The time of postoperative gastrointestinal function recovery, the number of days of physical recovery after the surgery, the average amount of bleeding in the repair process, and the average postoperative drainage in the laparoscopy group were significantly better than those in the open surgery group （P〈O.05）. Conclusion Laparoscopie surgery and open surgery in the treatment Of gastric perforation repair both have a certain effect, but laparoscopic repair of gastric perforation has a better effect and less trauma,the volume of bleeding is less, and the postoperative drainage is reduced,the recovery of gastrointestinal function and physical function is fast, with less complications,it is worthy of popularization.
