摘要 目的:探讨医护合作式健康教育联合行为干预对慢性阻塞性肺疾病(COPD)患者的影响。方法:选择116例呼吸内科住院的COPD患者采用数字表随机法分为对照组和研究组各58例,对照组采用常规护理,研究组在常规护理的基础上给予医护合作式健康教育联合行为护理;两组出院后均定期随访,采用COPD患者评估测试(CAT)量表评价两组生活质量,评价治疗(氧疗)依从性,同时比较两组12个月戒烟率、再入院率和住院时间。结果:研究组依从性良好率明显高于对照组(P<0.01);出院时两组症状、活动能力和疾病影响评分均较入院时明显降低(P<0.05),且研究组降低幅度明显大于对照组(P<0.05);研究组戒烟率高于对照组、再入院率明显低于对照组(P<0.05);12个月住院时间明显短于对照组(P<0.05)。结论:医护合作式健康教育联合行为干预可提高患者认知和依从性,改变不良的生活行为方式,从而减少复发,提高患者生活质量。 Objective : To explore the effect of medical cooperative health education combined behavioral intervention on chronic obstructive pulmonary disease (COPD). Methods:A total of 116 cases of COPD patients hospitalized in the respiratory department by randomly divided into the control group and the study group with 58 cases in each group. The control group was given routine nursing care, while the study group were treated with the medical cooperative health education combined behavioral intervention on the basis of routine nursing care. Two groups were followed up regularly after discharge, patients were assessed by COPD test(CAT) scale to evaluate the quality of life and the treatment (oxygen)compliance. 12 months smoking cessation rate, readmission rate and hospitalization time were compared between two groups. Results :The study group's good compliance rate was significantly higher than that in the control group( P 〈 0. 01 ). After discharge, two groups' symptoms and activity abilities and disease affect scores were significantly lower than those on admission( P 〈 0.05 ) , and the study group decreased significantly than those of the control group ( P 〈 0.05 ). The study group's smoking cessation rate was higher than that in the control group, and the study group's readmission rate was significantly lower than that in the control group ( P 〈 0.05 ). The hospitalized time during 12 months was significantly shorter than that in the control group (P 〈 0.05 ). Conclusion:The medical cooperative health education combined behavioral intervention can improve patients' cognition and compliance, and change the bad life behavior, so as to reduce the recurrence and improve the quality of life of patients.
