全方位护理在慢性阻塞性肺疾病并发Ⅱ型呼吸衰竭患者中的应用效果

Application effect of comprehensive nursing in patients with chronic obstructive pulmonary disease complicated with Ⅱ-respiratory failure

目的探讨全方位护理对慢性阻塞性肺疾病(COPD)并发Ⅱ型呼吸衰竭(Ⅱ-RF)患者中的应用效果。方法选择2013年1月~2016年1月我院收治的COPD并发Ⅱ-RF患者102例,按护理方法不同分为对照组和观察组,各51例。两组均进行控制感染、化痰、解痉平喘、营养支持、纠正酸碱失衡及应用Bi PAP呼吸机等常规治疗,同时对照组给予常规护理,观察组给予全方位护理。比较两组的护理满意度评分、平均住院时间及半年复发率情况。结果观察组护理满意度评分为(98.2±1.5)分,对照组护理满意度评分为(82.6±3.9)分,差异有统计学意义(P<0.05)。观察组平均住院时间为(28.56±2.92)d,对照组平均住院时间为(36.39±6.89)d,差异有统计学意义(P<0.05)。观察组6个月复发率为13.7%,显著低于对照组的27.5%,差异有统计学意义(P<0.05)。结论对COPD并发Ⅱ-RF患者实施全方位护理,可以明显提高护理满意度,对于促进疾病康复、降低复发率具有重要意义。

Objective To explore the application effect of comprehensive nursing in patients with chronic obstructive pulmonary disease（COPD） complicated with l/-respiratory failure（Ⅱ-RF）.Methods 102 patients with COPD complicated with Ⅱ-RF from January 2013 to January 2016 in our hospital were divided into control group and observation group according to different nursing methods, 51 cases in each group.Both groups patients were treated infection control,expectorant,antispasmodic,correct acid-base balance,nutrition,using BiPAP ventilator and other conventional treatment,the control group was given routine nursing,the observation group was given comprehensive nursing.Nursing satisfaction score,average hospitalization time and half year relapse rate were compared between the two groups.Results The nursing satisfaction score in observation group was （98.2±1.5）points,that was （82.6±3.9）points in control group,and the difference was statistical significance（P〈0.05）.The average hospitalization time in observation group was （28.56±2.92）days, that was （36.39±6.89）days in control group,and the difference was statistical significance（P〈O.05）.The 6 months relapse rate in observation group was 13.7%,was significantly lower than that in control group of 27.5%,and the difference was statistical significance （P〈0.05）.Conclusion Comprehensive nursing can improve the nursing satisfaction of the COPD complicated with Ⅱ-RF patients satisfaction degree of nursing,which promotes the recovery of disease and reduces the recurrence rate significantly.
