摘要
为探讨不同剂量的舒芬太尼对中老年胸科手术患者苏醒的影响,选择全身麻醉下行胸科手术的中老年患者100例,将所有患者随机分成A、B、C、D 4组,设定D组为对照组,手术结束关胸前不进行舒芬太尼注射;A、B、C 3组分别在手术结束关胸前静脉注射0.10μg/kg、0.15μg/kg和0.20μg/kg的舒芬太尼,对比4组患者麻醉前、麻醉后关胸时心率变化情况,记录四组患者手术过程中丙泊酚的用量,并对术后拔管时间、患者苏醒时间、患者自主恢复时间、术后躁动及呼吸抑制情况进行统计对比分析。结果显示,4组患者麻醉前心率差异不显著,在麻醉后关胸时D组的心率变化情况最大;A、B、C 3组患者术后拔管时间、苏醒时间、自主恢复时间相比于D组患者显著缩短;B、C两组患者术后躁动发生率相比于A、D两组显著降低;A和B组患者术后未出现呼吸抑制的情况。不同剂量的舒芬太尼对中老年胸科手术患者苏醒的影响也不相同,选择0.15μg/Kg更利于患者的术后苏醒,患者呼吸抑制情况发生较少,效果较好。
In order to study the effects of different doses of sufentanil on analepsia in elderly patients with thoracic surgery, 100 cases of elderly patients with thoracic surgery under general anesthesia were selected. All patients were randomly divided into A, B, C, D four groups, and group D was set as the control group. Before the sternum closing after surgery, A, B, C three groups were intravenously injected with 0.10μg/kg, 0.15 μg/kg and 0.20 μg/kg sufentanil, but group D was not. The heart rate changes at sternum closing after anesthesia and that before anesthesia of the four groups were compared, the amount ofpropofol used in the operation of the four groups were recorded, and the postoperative extubation time, analepsia time, patients' self recovery time, postoperative agitation and respiratory depression were statistically compared and analyzed. The results showed that the four groups of patients had no significant difference in heart rate before anesthesia, and heart rate changes of D group was the largest when at the sternum closing after anesthesia; The postoperative extubation time, analepsia time and patients' self recovery time of A, B and C groups were obviously shorter than D; The incidence of postoperative agitation of B and C groups were significantly lower than that of A and D groups; There was no respiratory depression in patients of A and B groups after operation. Different doses of sufentanil has different effects on the analepsia of patients with thoracic surgery, and dose of 0.15 μg/kg is more conducive to the analepsia of patients after surgery with which patients have less respiratory depression and better effect.
