摘要
目的检测树突状细胞(DC)及其各亚群[髓样树突状细胞(mDC)和浆细胞样树突状细胞(pDC)]在结核性胸膜炎(TP)患者外周血及胸腔积液中的变化,并且分析其与白细胞介素-12(IL-12)水平的相关性,探讨其临床意义。方法以12例健康志愿者作为对照组,采用流式细胞术对16例TP患者(TP组)外周血和胸腔积液中总体树突状细胞(tDC)及其各亚群的比例进行检测;同时,用ELISA法对TP患者外周血浆和胸腔积液中IL-12的含量进行检测。结果 TP组患者外周血中tDC和mDC比例显著高于对照组,治疗后tDC比例下降明显;TP组患者胸腔积液中tDC和mDC比例高于外周血,但pDC低于外周血;TP组患者血浆中IL-12的含量显著高于对照组,而胸腔积液中IL-12的含量则明显高于其血浆含量;TP患者血浆中IL-12的水平与外周血tDC、mDC呈正相关,并且胸腔积液中IL-12的水平也与胸腔积液tDC、mDC呈正相关。结论DC,尤其是mDC在TP中具有重要的抗结核免疫作用。
Objective To detect the dendritic cell (DC) subsets in peripheral blood and in pleural effusion of patients with tuberculous pleurisy (TP) , and to analyze the correlation of DC subsets with the level of IL - 12, for exploring its clinical significance. Methods Peripheral blood and pleural effusion from 16 cases of TP patients and 12 cases of healthy subjects ( HC group) were collected. Frequencies of DC subsets were analyzed by flow cytometry, while the enzyme linked immunosorbent assay (ELISA) was used to detect IL - 12 level in serum and pleural effusion. Results Frequencies of total DC (tDC) and myeloid DC (mDC) in peripheral blood of TP patients were higher than those in HC group, which were obviously decreased after treatment. Moreover, The ratios of tDC and mDC in pleural effusion of pa- tients were higher than those in peripheral blood. However, the frequency of plasmacytoid DC (pDC) in pleural effusion was lower than that in the peripheral blood. Interestingly, IL - 12 in pleural effusion and plasma from the TP patients were significantly higher than that in plasma from the HC group, and it is significantly higher in pleural eftusion than that in blood in same individual. Correlation analysis found that IL - 12 level was positively correlated with tDC and mDC in plasma as well as in pleural effusion. Conclusion DCs, especially mDC, plays an important role in anti -tuberculosis immune in tuberculous pleurisy.
