自拟白术益脾汤联合四联疗法治疗Hp阳性消化性溃疡的效果及对创面愈合胃酸分泌功能及复发情况的影响

Effect of Self-made Baizhu Yipi Decoction Combined with Quadruple Therapy for Treatment of Hp Positive Peptic Ulcer and its Effects on Wound Healing,Gastric Acid Secretion Function and Recurrence

目的:观察自拟白术益脾汤联合四联疗法治疗幽门螺杆菌(Hp)阳性消化性溃疡的效果及对创面愈合、胃酸分泌功能及复发情况的影响。方法:本次研究病例均来源于2012年3月~2014年5月我院门诊部收治的70例Hp阳性消化性溃疡患者,依据入院顺序将患者分为研究组和对照组各35例,对照组给予四联疗法治疗,研究组另联合自拟白术益脾汤治疗,对两组治疗后创面愈合、胃酸分泌功能[基础胃酸分泌量(BAO)、最大胃酸分泌量(MAO)、高峰胃酸分泌量(PAO)]、Hp根除及复发情况进行观察比较。结果:治疗后研究组创面愈合总有效率80.00%较对照组57.14%显著高(χ~2=4.242,P<0.05);两组治疗后BAO、MAO、PAO分别均较治疗前显著降低,且治疗后研究组较对照组降低显著,具有统计学意义(P<0.05);两组治疗后6个月、1年随访观察发现研究组Hp根除率分别均较对照组显著高,复发率显著低,均具有统计学意义(P<0.05);药物不良反应两组间比较无显著差异(P>0.05)。结论:自拟白术益脾汤联合四联疗法对Hp阳性消化性溃疡患者的临床疗效显著,有效促进创面愈合及Hp根除,并抑制胃酸分泌,降低复发率,是一种值得临床推广应用的药物方案。

Objective： To observe the effect of using Self-made Baizhu Yipi Decoction combined with quadruple therapy in the treatment of helicobacter pylori （Hp） positive peptic ulcer and its effects on wound healing, gastric acid secretion function and recurrence. Methods： 70 patients with Hp positive peptic ulcer treated in outpatient department of our hospital from March 2012 to May 2014 were enrolled in the study, patients were divided into study group and controlled group according to the order of admission （35 cases in each group） , the controlled group received quadruple therapy, the study group added Self-made Baizhu Yipi Decoction, wound healing, gastric acid secretion function [ basal acid output （ BAO）, maximal acid output （ MAO）, peak acid output （PAO） ] , Hp eradication and recurrence in the two groups after the treatment were observed and compared. Results： The total effective rate of wound healing in the study group after the treatment （ 80. 00% ） was significantly higher than the con- trolled group （57.14%） （ X^2 =4. 242, P 〈 0. 05） ; BAO, MAO, PAO after the treatment in the two groups were significantly de- creased than those before the treatment, the study group after the treatment were significantly decreased than the controlled group （ P 〈 0. 05 ） ; 6 month after the treatment and 1 year follow-up observation in the two groups showed that Hp eradication rate in the study group was significantly higher than the controlled group, the recurrence rate was significantly lower （ P 〈 0.05 ） ; there was no significant difference in drug adverse reactions between the two groups （ P 〉 0. 05 ）. Conclusion ： The clinical efficacy of using Self-made Baizbu Yipi Decoction combined with quadruple therapy in the treatment of Hp positive peptic ulcer is significant, can effectively promote wound healing and Hp eradication, inhibit gastric acid secretion, reduce recurt：ence rate, is a kind of drug regimen which is worth of spreading.
