摘要
目的探讨不同路面环境下认知负荷对帕金森病患者步态的影响。方法 9名帕金森病患者和9名健康对照受试者参与本研究。在不同路面条件(平坦与非平坦路面)和结构(水平和横向倾斜10°)下收集自由行走状态,步行同时执行计算任务状态下的步态时空参数。结果相比于自由行走,认知负荷条件下帕金森病患者组和健康对照组的步速、步频和步长均显著降低。非平坦路面对帕金森病患者组和健康对照组的时空参数均有影响,但对帕金森病患者的步态影响更加明显,表现为步宽增大和单双支撑期变化等特点。结论计算任务和路面条件的改变对帕金森病患者的步态时空参数影响显著,复杂路面环境和认知负荷可能会增加此类人群的跌倒风险。
Objective To evaluate the impact of cognitive load on gaits of patients with Parkinson's disease(PD) in different terrain conditions. Methods Nine PD patients and nine control subjects were recruited. Gait temporal parameters were collected from different terrain conditions(even and uneven) and configurations(level and 10 degree cross-slope) in free walking and walking with executing a calculation task. Results Comparing with free walking,the velocity,cadence and step length were significantly lower in walking with calculation task for both PD and control groups. Although the uneven terrain affected the gait temporal parameters for both PD and control groups,this uneven terrain effect was much more significant for PD group,which was verified by the larger step width and varied single and double support durations. Conclusions The gait temporal parameters are significantly affected by the calculation task and the change of terrain conditions for PD patients. The complex terrain and cognitive load may increase the possibility of fall risk for this population.
