腹腔镜手术联合药物治疗子宫内膜异位症伴不孕症对卵巢功能及妊娠结局的影响研究

Laparoscopic Surgery Combined with Medicine for the Treatment of Endometriosis Complicated with Infertility:Influence on the Ovarian Function and Pregnancy Outcome

目的探讨宫内膜异位症合并不孕症运用药物与腹腔镜手术治疗的临床效果。方法选择2013年5月至2015年5月期间我院收治的子宫内膜异位症合并不孕症患者106例为研究对象,随机分为两组各53例。对照组给予腹腔镜手术治疗,观察组给予腹腔镜手术联合孕三烯酮治疗,比较两组患者的妊娠结局和卵巢功能。结果治疗前,两组的血清E2和FSH水平比较无统计学差异(P>0.05);治疗后,观察组的血清FSH和E2水平均显著低于对照组,差异有统计学意义(P<0.05)。观察组的妊娠率和足月产率均显著高于对照组,流产率和早产率显著低于对照组,差异有统计学意义(P<0.05)。结论采用腹腔镜手术联合药物治疗子宫内膜异位症合并不孕症患者,不仅可以改善卵巢功能,还能提高妊娠率,值得临床推广。

Objective To explore the clinical effect of laparoscopic surgery combined with medicine for the treatment of endometriosis complicated with infertility. Methods 106 cases of patients with endometriosis complicated with infertility admitted to our hospital from May 2013 to May 2015 were selected and randomly divided into two groups,with 53 cases in each group. The control group received laparoscopic surgery,while the observation group received laparoscopic surgery combined with gestrinone therapy. The ovarian function and pregnancy outcome were compared between two groups. Results Before treatment,no statistical difference was found between two groups in the levels of serum E2 and FSH（P〉0.05）; After treatment,the observation group had significantly lower levels of serum E2 and FSH than the control group（P〈0.05）. The pregnancy rate and full-term delivery rate of observation group were significantly higher than those of control group,and the abortion rate and premature birth rate were significantly lower than those of control group（all P〈0.05）.Conclusions Laparoscopic surgery combined with medicine for the treatment of endometriosis complicated with infertility can improve patients＇ ovarian function and improve the pregnancy rate,which is worthy of clinical promotion.
