肺炎支原体IgM及白细胞检测在儿童呼吸道感染中的应用 被引量:5

摘要 目的探讨肺炎支原体IgM及白细胞(WBC)检测在儿童呼吸道感染中的临床价值。方法选取2015年11月至2016年10月于我院就诊的586例呼吸道感染儿童作为研究对象,患儿就诊后均完成肺炎支原体IgM和血常规检测,比较肺炎支原体IgM检测阳性患儿的性别、年龄分布,根据患儿最终结果明确感染类型,比较细菌感染、肺炎支原体感染及病毒感染患儿中肺炎支原体IgM及WBC的阳性率。结果 586例呼吸道感染患儿经检测,肺炎支原体IgM阳性146例,阳性率为24.91%。146例阳性患儿中,男女阳性率比较无统计学差异(P>0.05);3~5岁患儿的阳性率显著高于0~2岁、>5岁患儿的阳性率,差异有统计学意义(P<0.05)。肺炎支原体感染患儿的肺炎支原体IgM阳性率显著高于细菌、病毒感染患儿,差异有统计学意义(P<0.05);细菌感染患儿的WBC阳性率显著高于病毒、肺炎支原体感染患儿,病毒感染患儿的WBC阳性率显著高于肺炎支原体感染患儿,差异有统计学意义(P<0.05)。结论不同致病菌呼吸道感染患儿的肺炎支原体IgM和WBC检测阳性率不同,临床中通过检测肺炎支原体IgM、WBC能快速初步鉴别呼吸道感染类型。 Objective To investigate the clinical value of detection of mycoplasma pneumonia IgM(MP-IgM) and white blood cell(WBC) in children with respiratory tract infection. Methods 586 cases of children with respiratory tract infection in our hospital from November 2015 to October 2016 were selected as research objects. All cases received MP-IgM detection and blood routine examination,and the distributions of gender and age of MP-IgM positive children were analyzed. According to the determined infection type,the positive rates of MP-IgM and WBC were compared among children with bacterial infection,mycoplasma pneumoniae(MP) infection and viral infection.Results Among 586 cases of children with respiratory tract infection,146 cases had positive MP-IgM,with the positive rate of 24.91%; The positive rate of MP-IgM showed no statistical difference between male and female cases(P〉0.05),while the positive rate of MP-IgM of children aged from 3-5 was significantly higher than that of children aged from 0-2 and children over 5 years old(P〈0.05). The positive rate of MP-IgM of children with MP infection was significantly higher than that of children with bacterial infection and children with viral infection(P〈0.05); The positive of WBC of children with bacterial infection was significantly higher than that of children with MP infection and children with viral infection,and the positive rate of WBC of children with viral infection was significantly higher than that of children with MP infection(P〈0.05). Conclusions The positive rates of MP-IgM and WBC are different in children with respiratory tract infection caused by different pathogens. The detections of MP-IgM and WBC can quickly identify the type of respiratory tract infection in clinic.
