摘要
目的探讨细致化护理对慢性阻塞性肺疾病(慢阻肺)急性发作患者不良心理应激的效果。方法选择我院收治的90例慢性阻塞性肺疾病患者,随机分为两组各45例。对照组采用常规护理,观察组在常规护理的基础上对常规护理进行细致化处理。观察和比较两组患者治疗前后的生存质量(SGRQ评分)、运动能力(6MWD值)及心境状态。结果 (1)两组患者治疗后的SGRQ评分均显著低于治疗前,6MWD值显著高于治疗前,差异有统计学意义(P<0.05);观察组治疗后的SGRQ评分显著低于对照组,6MWD值显著高于对照组,差异均有统计学意义(P<0.05)。(2)观察组的紧张、抑郁、疲乏、愤怒、迷惑、精力评分均显著低于对照组,自尊感评分显著高于对照组,差异有统计学意义(P<0.05)。结论细致化护理对慢阻肺急性发作患者的不良心理应激具有显著效果,能够有效地缓解患者的不良情绪,改善患者的生存质量和运动能力,值得临床广泛应用以及推广。
Objective To explore the effect of detailed nursing on the adverse psychological stress in patients with chronic obstructive pulmonary disease at the stage of acute exacerbation. Methods 90 cases of patients with chronic obstructive pulmonary disease at the stage of acute exacerbation admitted to our hospital were selected and randomly divided into the control group and the observation group,with 45 cases in each group. All cases received routine nursing,while the observation group received detailed nursing additionally.The quality of life(SGRQ score) and exercise ability(6MWD value) before and after treatment,and mood state were observed and compared between two groups. Results(1)After treatment,both groups had significantly decreased SGRQ score and increased 6MWD value compared with those before treatment(intra-group comparison,P〈0.05),and the observation group had lower SGRQ score and higher 6MWD value than the control group(intergroup comparison,P〈0.05).(2) The scores of tension,depression,fatigue,anger,confusion and energy of observation group were significantly lower than those of control group,while the score of self-respect was significantly higher than that of control group,with statistical differences(all P〈0.05). Conclusions Detailed nursing on the adverse psychological stress in patients with chronic obstructive pulmonary disease at the stage of acute exacerbation has significant effect,which can effectively relieve patients' unhealthy emotion,and improve patients' quality of life and exercise ability,which is worthy of clinical promotion and application.
