掌背动脉岛状皮瓣修复指背皮肤缺损54例

Dorsal digital dorsal artery island flap for repairing finger skin defect in 54 cases

目的探讨掌背动脉岛状皮瓣修复与腹部皮瓣修复指背皮肤缺损的临床疗效。方法选取湖北医药学院附属东风医院2013年12月~2015年12月收治的指背皮肤缺损患者108例,根据是皮瓣修复方法,将患者分为对照组(54例,患指56根)和实验组(54例,患指55根);对照组行腹部皮瓣修复,实验组行掌背动脉岛状皮瓣修复。比较两组手术时间、术后住院费用以及住院天数;随访6个月后,比较两组术后效果。结果手术情况:实验组患者的住院费用少于对照组,住院时间短于对照组,差异有高度统计学意义(P<0.01);两组手术时间比较,差异无统计学意义(P>0.05);术后效果:随访6个月后,实验组优良率达94.64%(53/56),高于对照组的72.72%(40/55),差异有高度统计学意义(P<0.01)。结论掌背动脉岛状皮瓣修复指背皮肤缺损具有较好的临床疗效,操作较为简单,在临床上易于推广。

Objective To explore the clinical efficacy of dorsal digital dorsal artery island flap and abdominal skin flap for repairing finger skin defect. Methods From December 2013 to December 2015, in Dongfeng Hospital Affiliated to Hubei University of Medicine, 108 patients with finger skin defect were selected and divided into control group（54 cases,54 wounded fingers） and experimention group（54 cases, 55 wounded fingers）, according to skin flaps repaired method. Control group was given abdominal skin flap repairing finger skin defect, experimention group was given dorsal digital dorsal artery island flap repairing finger skin defect. The operation time, postoperative hospitalization expenses and hospitalization days of two groups were compared. After 6 months follow-up, the effect of the two groups was compared. Results Operation situation： hospitalization expenses and hospitalization days of experimention group were less than those of control group, the differences were statistically significant（P 〈 0.01）, but the operation time of two groups were compared, the difference was not statistically significant（P 〉 0.05）. Postoperative effect： after followed up for 6 months, the excellent and good rate of experimention group was 94.64%（53/56）, which was higher than that of control group [72.72%（40/55）], the difference was statistically significant（P 〈 0.01）. Conclusion Dorsal digital dorsal artery island flap for repairing of finger skin defect has good clinical curative effect and simple operation. It is easy to be popularized in clinic.
