2型糖尿病患者糖尿病痛苦的现状及其影响因素分析

Diabetes Distress of Patients with Type 2 Diabetes Mellitus and Its Influence Factors

目的了解2型糖尿病患者糖尿病痛苦的现状,并分析其影响因素。方法采用一般资料调查问卷、糖尿病痛苦量表和疾病不确定感量表,对大连市某三级甲等医院250例2型糖尿病患者进行问卷调查。采用多元逐步回归分析2型糖尿病患者糖尿病痛苦的影响因素。结果 2型糖尿病患者糖尿病痛苦总均分为(2.49±0.83)分,疾病不确定感总分为(96.54±11.67)分。糖尿病痛苦与疾病不确定感呈正相关(r=0.350,P<0.01);多元逐步回归分析结果显示:文化程度、病程、疾病不确定感是2型糖尿病患者糖尿病痛苦的主要影响因素(P<0.05)。结论 2型糖尿病患者糖尿病痛苦处于中等水平,疾病不确定感现状有待改善,患者的文化程度、病程、疾病不确定感是其影响因素。医护人员应重视患者疾病不确定感现状,并根据影响因素,制定针对性的干预措施,从而降低患者的糖尿病痛苦水平,提高患者的生活质量。

Objective To investigate the current status and influence factors of diabetes distress among the type 2 diabetes patients. Methods General information questionnaire, Diabetes Distress Scale（DDS） and Mishel＊s Uncertainty in illness Scale-adult（MUIS-A） were used to survey 250 patients with type 2 diabetes in a class Ⅲ grade Ⅰhospital in Dalian city.Multiple stepwise regression was used to analysis influence factors of diabetes distress in them. R e su lts The average score of diabetes distress of patients with type 2 diabetes was 2.49 ±0.83 and the total score of uncertainty in illness 96.54 ±11.67. The diabetes distress was positively related with patient’s uncertainty in illness （r=0.350, H〈0.01）. Multiple stepwise regression showed that the educational background, course of disease, uncertainty in illness were the main influence factors of diabetes distress （H〈0.05）. Conclusion The patients’ diabetes distress was in a middle level and patients ’ uncertainty in illness needs to be further improved. Educational levels, course of disease, uncertainty in illness were the influence factors of diabetes distress. Healthcare workers should pay attention to the current situation of patients’ uncertainty in illness. Targeted interventions based on the influence factors of diabetes distress are beneficial to reducing diabetes distress in patients with type 2 diabetes and improving their quality of life.
