无创呼吸机治疗急性心力衰竭并呼吸衰竭患者的临床疗效研究

Clinical Effect Study of Noninvasive Ventilator Treated on Acute Heart Failure with Respiratory Failure

目的探讨无创呼吸机治疗急性心力衰竭并呼吸衰竭患者的临床疗效。方法选取2014年3月—2016年9月泸县人民医院ICU收治的急性心力衰竭合并呼吸衰竭患者84例,采用随机数字表法将患者分为对照组和治疗组,每组42例。对照组患者给予常规对症治疗,治疗组患者在此基础上给予无创呼吸通气治疗。观察两组患者临床疗效,并比较两组患者治疗前后血气分析指标。结果治疗前两组患者心率、呼吸频率、p H值、血氧饱和度(Sp O2)、氧分压(Pa O2)、二氧化碳分压(Pa CO2)比较,差异无统计学意义(P>0.05);治疗后治疗组患者心率、呼吸频率、Pa CO2低于对照组,p H值、Sp O2、Pa O2高于对照组(P<0.05)。治疗组患者病情好转时间及住院时间短于对照组,临床疗效优于对照组(P<0.05)。结论无创呼吸机治疗急性心力衰竭合并呼吸衰竭患者的临床疗效确切,能有效改善患者血气分析指标,促进患者康复。

Objective To investigate the clinical effect of noninvasive ventilator treated on acute heart failure with respiratory failure. Methods A total of 84 cases of acute heart failure with respiratory failure patients were selected from March2014 to September 2016 in ICU of People＇s Hospital of Luxian County,which were divided into control group and treatment group according to random number table method,42 cases in each group. The control group was given routine symptomatic treatment,treatment group was given noninvasive ventilator on the basis of control group. The clinical effect,blood gas analysis index before and after treatment were observed and compared. Results Before treatment,no statistically significant differences of heart rate,respiratory rate, p H value, Sp O2,Pa O2,Pa CO2 was found between the two groups（ P 〉 0. 05）; after treatment, the treatment group of heart rate,respiratory rate,Pa CO2 were lower than control group,p H value,Sp O2,Pa O2 were higher than control group（ P〈 0. 05）. The improved time of symptoms and hospitalization time were shorter than control group（ P 〈 0. 05）.Conclusion Noninvasive ventilator has an obviously clinical effect treated on acute heart failure with respiratory failure,it can effectively improve the blood gas analysis index,and promote the rehabilitation of patients.
