双轨互动护理干预模式在社区糖尿病患者管理中的应用效果评价

Effect of Double-track Interactive Nursing Intervention Model in the Management of Diabetes Patients in Community

目的评价双轨互动护理干预模式在社区糖尿病患者管理中的应用效果。方法选取2014年10月—2016年5月在深圳市龙华新区人民医院三联社区健康服务中心就诊的糖尿病患者60例,随机分为对照组和干预组,各30例。对照组采用常规护理方式,干预组采用双轨互动护理干预模式。比较两组患者血糖控制达标率、干预前后焦虑自评量表(SAS)评分及干预后患者行为改变评分。结果干预组患者血糖控制达标率高于对照组(P<0.05)。干预前两组患者SAS评分比较,差异无统计学意义(P>0.05);干预后干预组患者SAS评分低于对照组(P<0.05)。干预后干预组患者规律饮食、遵医服药、坚持运动、情绪控制、胰岛素使用评分高于对照组(P<0.05)。结论双轨互动护理干预模式能有效提高社区糖尿病患者的血糖控制达标率,缓解患者焦虑情绪,有效改善患者的日常行为。

Objective To explore the effect of interactive nursing intervention model in the management of diabetes patients in community. Methods From October 2014 to May 2016,60 cases diabetes patients were selected in Sanlian Community Health Service Center of Longhua New Area People’ s Hospital, Shenzhen City, they were randomly divided into control group and intervention group, each 30 cases. Control group were accepted conventional care, intervention group were accepted dual-track interaction nursing intervention model. The blood glucose control compliance rate,the self rating Anxiety Scale（ SAS） score before and after intervention,and the score of behavioral change after intervention were compared between the two groups. Results The blood glucose control compliance rate of intervention group was statistically significantly higher than that of control group（ P 〈 0. 05）. No significantly difference in SAS score between the two groups before intervention（ P 〉0. 05）; the SAS score of intervention group was statistically significantly lower than that of control group after intervention（ P 〈0. 05）. After intervention, the scores of regular diet, medication compliance, adherence to exercise, emotional control,insulin use of intervention group were statistically significantly higher than those of control group（ P 〈 0. 05）. Conclusion Double-track interactive nursing intervention model can effectively improve the rate of diabetes patients in the community to control the rate of compliance,to ease the anxiety of patients,effectively improve the patient’s daily behavior.
