社会网络密度在组建社区高血压自我管理小组中的效果分析

目的分析相同干预措施下,依据社区高血压患者社会网络关系紧密程度组建自我管理小组的效果。方法采用分阶段抽样的方法抽取北京市西城区4个社区卫生服务站,招募293名社区高血压患者进入研究并签署知情同意书。依据患者相对度数中心度分为紧密组148人和不紧密组145人。在两组进行内容相同的8次课程干预,干预前后由调查员统一完成调查,调查内容包括基本情况,知识、态度、行为、技能和体格检查,比较两组的干预效果。结果在制定行动计划、锻炼两个指标上,紧密组和不紧密组干预后得分的差异有统计学意义(P<0.05);紧密组舒张压在干预后的改善优于不紧密组(P<0.05)。在各条目的比较中,知道控制情绪的方法,运动时出现气急会慢下来,就诊前会整理问题,紧密组干预后的改善优于不紧密组(P<0.05)。结论依据社区高血压患者社会网络密度建立管理小组干预效果较好,可作为社区高血压自我管理小组的构建方式。

Objective To analysis the effect of social network density on hypertension self-management group in the community under the same intervention. Methods Four community health service stations were extracted with multi-stage sampling method,293 hypertension patients were observed and signed informed consent. Patients were divided into strong-tie group（ 148） and weak-tie group（ 145） according to the degree centrality. Eight same intervention courses were contented. Surveys were completed by investigators before and after intervention. The survey included basic situations,knowledge,attitude,behavior,skills and physical examination. The effects were analyzed by SPSS 16. 0. Results According to the two indexes（ action planning and exercising）,the scores after intervention between the strong-tie group and weak-tie group were statistically significant（ P 〈 0. 05）. The improvement of diastolic blood pressure of the strong-tie group was better than that of the weak-tie group（ P 〈 0. 05）. The comparison between the individual items showed that the understanding of how to control emotion,to slow down when short of breath occurred during exercising,and sorting out problems before treatment,the differences between strong-tie group and weak-tie group were statistically significant（ P 〈 0. 05）. Conclusion According to the social net density of hypertension patients in the community,the set up of the management group would have better intervention outcome,this could be a mode for constructing the hypertension self-management group in the community.
