医护联合家访健康教育干预对老年出院病人护理延伸服务效果评价

Evaluation on the Effect of Health Education Intervention on Nursing Extension Service of Elderly Discharged Patients

目的:探讨评价医护联合家访健康教育干预对老年出院病人护理延伸服务的效果。方法:我院护理部联合临床医生组成了出院病人家访服务小组,对2013年1~12月份我院各临床科室的部分老年出院病人共100例(观察组)进行医疗服务满意度调查及健康教育干预家访服务。并与2012年1~12月临床科室部分老年出院病人108例(对照组)问卷调查结果进行对比分析。结果:观察组与对照组在健康知识普及率及满意度问卷调查结果方面存在明显差异,有统计学意义(P<0.05)。结论:医护联合家访健康教育干预能积极推进现代化医院倡导的院前、院中、院后一体化医疗护理服务模式,将医疗护理服务延伸至患者出院后,大大提高社会、患者对医院的满意度,树立了医院良好的形象,不仅架起了医患沟通的桥梁,化解了医患矛盾,同时也使医疗护理服务从医院延伸到了社区及患者的家里,尤其适于在老龄患者中推广应用。

Objective： To explore the effect of health education intervention on the extension of nursing care for elderly pa-tients. Methods： Our hospital nursing department joint clinician composed of patients discharged from the patient service group, from January to December 2013 our hospital clinical department of some elderly patients discharged from a total of 100 cases （observation group） for medical service satisfaction survey and health education intervention family visit service. And compared with the questionnaire survey of 108 cases （control group） of some elderly patients in clinical department from Janu-ary to December in 2012. Results ： There was significant difference between the observation group and the control group in the health knowledge penetration rate and satisfaction questionnaire （P 〈C 0. 05）. Conclusions： Health care joint home health edu-cation intervention can actively promote the modern hospital advocated by the hospital, hospital, hospital after the integration of medical care service model,the medical care services extended to patients discharged,greatly improve the community, the patient satisfaction with the hospital. Establish a good image of the hospital, not only set up a bridge between doctors and pa-tients to resolve the contradiction between doctors and patients but also to medical care services extended from the hospital to the community and the patients home, especially for the promotion and application of elderly patients.
