小儿手足口病患儿血清学指标检测的临床意义

Clinical significance of serological index detection in children with hand foot and mouth disease

目的:观察小儿手足口病患儿血清学指标检测的临床意义。方法:选取220例手足口病患儿,依据患儿病情分为普通型组(115例)、重型组(62例)和危重型组(43例);同时,将收治的208例健康儿童设为对照组。对4组研究对象进行血清ɑ-羟丁酸脱氢酶(ɑ-HBDH)、肌酸激酶(CK)、肌酸激酶同工酶MB(CK-MB),皮质醇(COR)、白介素13(IL-13)和干扰素-γ(IFN-γ)及放射免疫法检测β内啡肽(β-EP)水平检测分析。结果:手足口病3组患儿的COR、β-EP、IL-13以及IFN-γ明显高于对照组(P<0.01),组间比较均有显著性差异(P<0.01);手足口病3组患儿的CK、CK-MB和心肌损害发生率指标均明显高于对照组(P<0.01),组间比较有明显差异(P<0.01);4组患儿的ɑ-HBDH指标比较无明显差异(P>0.05)。结论:开展小儿手足口病患儿血清学指标检测对其临床诊治具有重要意义。

Objective： To observe clinical significance of serological index detection in children with hand foot and mouth disease. Methods： 220 children with hand foot and mouth disease were selected and divided into normal group（ n = 115）,severe group（ n= 62） and critical group（ n= 43） according to their conditions. At the same time,208 healthy children were used as control group.The levels of ɑ-hydroxybutyrate dehydrogenase（ ɑ-HBDH）,creatine kinase（ CK）,Creatine kinase isoenzyme（ CK-MB）,cortisol（ COR）,interleukin-13（ IL-13）,interferon-γ（ IFN-γ） and β-endorphin（ β-EP,measured by radioimmunoassay） were detected and analyzed. Results： The levels of COR,β-EP,IL-13 and IFN-γ of the three hand foot and mouth groups were significantly higher than those of control group（ P〈0.01）,and the differences among these groups were significant（ P〈0.01）. The levels of CK and CKMB and the incidence of myocardial damage of the three hand foot and mouth groups were significantly higher than those of control group（ P〈0.01）,and the differences among these groups were significant（ P〈0.01）. Moreover,the differences in the ɑ-HBDH level among the four groups were not significant（ P〈0.05）. Conclusions： The serological index detection has an important significance in the treatment of the children with hand foot and mouth disease.
