钢板与记忆合金内固定治疗复杂髋臼骨折疗效分析

Complex acetabular fracture treatment between plate and memory alloy internal fixation

目的探讨钢板与记忆合金内固定治疗复杂髋臼骨折的临床疗效。方法回顾性分析自2006年1月至2014年1月沈阳军区总医院收治的145例复杂髋臼骨折合并骨缺损患者,其中,应用钢板内固定81例(钢板组),记忆合金内固定64例(记忆合金组)。采用改良Matta评分对两组患者疗效进行评估及比较。结果全部患者均获骨性愈合。钢板组,优良率88.9%,远期并发症发生率12.3%;记忆合金组,优良率89.1%,远期并发症发生率12.5%。两组改良Matta评分和并发症发生率比较,差异均无统计学意义(P>0.05)。结论复杂髋臼骨折的切开复位内固定手术依然是骨科领域较为棘手的问题,应用钢板螺钉系统和记忆合金系统治疗复杂髋臼骨折均能够获得良好的临床疗效。

Objective To investigate the curative effects of complex acetabular fracture between plate and memory alloy internal fixation. Methods A retrospective study was performed on 145 cases of complex acetabular fracture who were admitted from January 2006 to January 2014. All patients were performed open reduction and internal fixation,81 cases were in the plate group and 64 cases were in the memory alloy group. Modified Matta score was used for curative effect evaluation.Results All cases were got bony cure. In the plate group,the percent of excellent and good was 88. 9%,and the percent of long-term complications was 12. 3%. In the memory alloy group,the percent of excellent and good was 89. 1%,and the percent of long-term complications was 12. 5%. There was no statistically significant difference of modified Matta score and complications between the two groups（P〈0. 05）. Conclusion Open reduction and internal fixation of complex acetabulum fracture surgery is still intractable problems in orthopaedic field. Plate and memory alloy internal fixation can achieve good results for complex acetabular fractures.
