摘要
目的探讨2型糖尿病(T2DM)患者血清胱抑素C(CysC)、尿足细胞标志蛋白podocalyxin(PCX)的表达及临床意义。方法选取邯郸市人民医院内科收治的T2DM患者130例为病例组,根据24h尿白蛋白定量(24h-UAE)水平将其分为3组:24h-UAE<30 mg 55例为正常蛋白尿亚组;24h-UAE 30~300 mg 40例为微量蛋白尿亚组;24h-UAE>300 mg 35例为大量蛋白尿亚组。另选同期健康体检者50例为对照组。比较各组血清Cys C、尿PCX水平。结果病例组患者血清Cys C水平、尿PCX水平均高于对照组,差异均有统计学意义(P<0.05)。T2DM患者大量蛋白尿亚组、微量蛋白尿亚组及正常蛋白尿亚组血清Cys C水平、尿PCX水平比较差异均有统计学意义(P<0.05)。T2DM患者血清CysC水平与尿PCX水平呈正相关(r=0.843,P<0.05)。结论血清Cys C、尿PCX是可以反映糖尿病早期肾损伤的实验室指标,对糖尿病肾损伤的早期诊断及疗效判断具有较好的临床应用价值。
Objective To study the level of serum Cys C and urinary podocalyxin in patients with type 2 diabetes melitus（ T2DM）,and analyze the association between the levels of serum Cys C and urinary podocalyxin and the renal injury.Methods Selected 130 cases patients with type 2 diabetic in medical department of People＇ s Hospital of Handan as patient group,according to the 24 h urine albumin quantitative（ 24 h-UAE） level,130 cases patients were divided into normal albuminuria group（ urinary albumin 30 mg /24 h,n = 55）,microalbuminuria group（ 〈30 ≤urinary albumin 〈300 mg /24 h,n = 40）,macroalbuminuria group（ urinary albumin 300 mg /24 h,n = 35）. Health check-up of 50 cases as the control group. Compared the levels of serum Cys C and urinary podocalyxin. Results The levels of serum Cys C and urine PCX were higher than control group,the differences were statistically significant（ P〈0. 05）. The levels of serum Cys C and urine PCX In patients with T2 DM group were statistically significant（ P〈0. 05）. The level of serum Cys C was positively correlated with urine PCX（ r = 0. 843,P〈0. 05） .Conclusion The levels of serum Cys C,urine PCX can be used as sensitive indicators for the diagnosis of early diabetic nephropathy in patients with diabetes mellitus,it is of great value to early diagnosis and treatment of diabetic nephropathy.
