红光照射联合中药塌渍和他克莫司软膏治疗面部糖皮质激素依赖性皮炎疗效观察

Curative effect of red light irradiation combined with Chinese medicine stains and tacrolimus ointment in treatment of facial steroid-dependent dermatitis

目的评价红光照射、中药塌渍和他克莫司软膏联合治疗面部糖皮质激素(简称激素)依赖性皮炎的疗效。方法将2014年3月至2015年9月在开封市第二人民医院皮肤科就诊的激素依赖性皮炎患者120例分为他克莫司组、中药塌渍组和联合治疗组,每组40例。他克莫司组患者给予他克莫司软膏外用;中药塌渍组患者给予中药塌渍联合他克莫司软膏治疗;联合治疗组患者给予他克莫司软膏、中药塌渍和红光照射治疗。各组患者均每日给药1次,连续治疗4周后进行临床疗效和不良反应评价。结果 3组患者治疗前症状积分比较差异均无统计学意义(P>0.05);治疗后3组患者症状积分均低于治疗前(P<0.05)。治疗后,联合治疗组患者症状积分低于中药塌渍组和他克莫司组(P<0.05);中药塌渍组患者症状积分低于他克莫司组(P<0.05)。他克莫司组、中药塌渍组和联合治疗组患者治疗有效率分别为52.5%、75.0%、95.0%,联合治疗组患者治疗有效率高于中药塌渍组和他克莫司组(P<0.05);中药塌渍组患者治疗有效率高于他克莫司组(P<0.05)。3组患者均未出现严重不良反应。结论红光照射、中药塌渍和他克莫司软膏联合治疗面部激素依赖性皮炎疗效确切,安全性好。

Objective To evaluate the effect of red light irradiation combined with Chinese medicine stains and tacrolimus ointment in treatment of facial steroid-dependent dermatitis. Methods One hundred and twenty patients with steroid-dependent dermatitis in Kaifeng Second People＇s Hospital from March 2014 to September 2015 were selected and divided into tacrolimus group,Chinese medicine stains group and combined treatment group,with forty patients in each group. The patients in tacrolimus group were treated with tacrolimus ointment; the patients in Chinese medicine stains group were treated with Chinese medicine stains and tacrolimus ointment; the patients in combined treatment group were treated with Chinese medicine stains,tacrolimus ointment and red light irradiation. All patients were treated once daily for 4 weeks,then the clinical efficacy and adverse reaction were evaluated. Results There were no statistic difference in symptom scores of the patients among the three groups before treatment（ P 〉 0. 05）. The symptom scores of the patients in the three groups after treatment were significantly lower than before treatment（ P 〈 0. 05）. After treatment,the symptom scores of the patients in the combined treatment group were significantly lower than those in Chinese medicine stains group and tacrolimus group（ P 〈 0. 05）; the symptom scores of the patients in the Chinese medicine stains group were significantly lower than those in tacrolimus group（ P 〈 0. 05）. The effective rate in tacrolimus group,Chinese medicine stains group and combined treatment group was 52. 5%,75. 0%,95. 0% respectively. The effective rate in combined treatment group was significantly higher than that in Chinese medicine stains group and tacrolimus group（ P 〈0. 05）; the effective rate in Chinese medicine stains group was significantly higher than that in tacrolimus group（ P 〈 0. 05）.There were no serious adverse reactions occurred in the three groups. Conclusion Red light irradiation combined with Chinese medicine stains and tacrolimus ointment for treatment of facial steroid-dependent dermatitis is effective and safe.
