小骨窗开颅血肿清除术和传统开颅血肿清除术治疗脑出血的疗效对比分析

Comparison of small bone window craniotomy evacuation of hematoma and traditional craniotomy evacuation of hematoma in the treatment of cerebral hemorrhage

目的对比小骨窗开颅血肿清除术和传统开颅血肿清除术用于脑出血（ICH）的治疗效果。方法将本院2009年6月至2015年6月期间收治的70例ICH患者，根据手术方式不同分为A组（35例）和B组（35例），分别采用小骨窗开颅血肿清除术和传统开颅血肿清除术治疗。比较两组的治疗效果。结果A组手术时间、术中出血量、住院天数和并发症发生率均显著低于B组（P〈0．05）；术后6个月A组CSS评分和BI指数改善程度显著优于B组（P〈0．05）；A组愈显率显著高于B组（88．57％比65．71％，P〈0．05）。结论小骨窗开颅血肿清除术治疗ICH疗效肯定，可作为ICH治疗的主要术式之一。

Objective To compare the clinical effect of small bone window craniotomy evacuation of hematoma and traditional craniotomy evacuation of hematoma in the treatment of cerebral hemorrhage （ICH）. Methods 70 cases of ICH in our hospital from June 2009 to June 2015 were divided into group A and B, 35 cases in each group. Patients in group A and B were given small bone window craniotomy evacuation of hematoma and traditional craniotomy evacuation of hematoma respectively. The treatment effect of two groups were compared. Results Operative time, blood loss, hospital stay, and the incidence of complications in group A were significantly less than those in group B （P〈0.05）. The improvement of CSS score and BI index at 6 months after operation in group A were significantly better than those in group B （P〈0.05）. The markedly effective rate of group A was significantly higher than that of group B （88.57% vs. 65.71%, P〈0.05）. Conclusion Small bone window craniotomy evacuation of hematoma is effective in treating ICH, can be used as the main operative treatment method for ICH.
