同期放疗联合内分泌治疗对中晚期前列腺癌患者生存质量的影响

Effect of concurrent radiotherapy combined with hormonal therapy on the quality of life in patients with advanced prostate cancer

目的探讨同期放疗联合内分泌治疗对中晚期前列腺癌患者生存质量的影响。方法选取中晚期前列腺癌患者82例,依据治疗方式的不同分为对照组(n=41)和研究组(n=41)。对照组行三维适形放射治疗,研究组行同期放射治疗联合内分泌治疗(口服比卡鲁胺50 mg,每天1次,皮下注射戈舍瑞林3.6 mg,每28 d 1次,持续3年)。比较两组患者放疗前和放疗后3年的最大尿流率、前列腺癌症状评分表(EPIC)评分及并发症发生情况。结果放疗后3年,研究组和对照组患者的最大尿流率分别为(13.67±1.68)ml/s和(11.03±0.98)ml/s,差异有统计学意义(P﹤0.05);研究组患者的EPIC评分高于对照组(P﹤0.05);研究组和对照组患者在治疗期间的并发症总发生率分别为41.5%和70.7%,差异有统计学意义(P﹤0.05)。结论同期放疗联合内分泌治疗能够改善中晚期前列腺癌患者的生理功能状况,减少不良反应,提高患者的生存质量,具有重要的临床应用价值。

Objective To explore the effect of concurrent radiotherapy combined with hormonal therapy on the quali-ty of life in patients with advanced prostate cancer. Method 82 cases of advanced prostate cancer were included, and were stratified as control group （n=41） or study group （n=41） according to respective therapy applied. The control group was given three-dimensional conformal radiation therapy, while the study group was given a combination therapy of con-current radiotherapy plus endocrine therapy （bicalutamide 50 mg, qd, and subcutaneous goserelin 3.6 mg, once every 28 d, 3 years）;the maximum urinary flow rate, EPIC scores before and after treatment, as well as the incidence of complica-tions in the two groups were observed and compared. Result In 3 years after radiotherapy, the study group had a maxi-mum urinary flow rate of （13.67±1.68） ml/s compared with （11.03±0.98） ml/s in the control group （P〈0.05）;and the EP-IC scores were also higher in study group than control group （P〈0.05）;a significant difference was observed for the over-all incidence of complications during treatment between study group and control group as 41.5%vs 70.7%（P〈0.05）. Con-clusion Concurrent radiotherapy combined with hormonal therapy improves the physiological function in patients with advanced prostate cancer, with fewer adverse reactions and better quality of life.
